Development Staff Comments:
1. Submit $225.00 filing fee (the filing fee includes the sign).
2. Provide notarized affidavit form.
3. Submit completed ROW dedication form.
4. Provide a bill of assurance. If the property is not located in a subdivision with a bill of
assurance, submit a statement to that effect to staff.
