No deviations from the accepted plans, specifications, and/or addenda will be permitted during construction except by prior written acceptance. This approval is valid for one (1) year from the date on this letter or this acceptance must be re- validated by contacting this office referring to the above referenced file numbers. All plumbing and gas work shall meet minimum state standards and be performed by a duly licensed master plumber. Please refer to any footnotes at the bottom of this letter regarding required changes or the need for additional plumbing. This approval is for the plumbing portion of this project only. Swimming pools, public water/sewer extensions, food service, fire protection systems, septic systems, and water wells are regulated by other agencies of the Arkansas Department of Health and are subject to plan review approval before construction and the inspection of systems after construction. The following people shall be contacted at least twenty-four (24) hours before construction. Inspection fees and municipal permits may be required. This project has been reviewed as a business occupancy, type IV, unprotected construction and not sprinkled. Response: We concur. 2. Written approval from the State Health Department Plumbing Division, must be obtained for all commercial buildings prior to issuance of the building permit. Contact Jim Ward at 661-2000 with any questions. Response: Plans have been submitted to the Health Department's Plumbing Division. 3. The tenant separation walls must be one -hour rated. Response: Agree. The tenant separations were built as separate buildings and a minimum of one hour should exist. During the construction, this rating will be verified and restored if necessary. 4. Mount the accessible commode 18 inches off the side wall and install 36" grab bar on the rear and a 42" grab bar on the side. Mount the accessible lavatory at 34 inches and provide 29-inch leg clearance. Insulate the drain and hot water lines and mount the mirror at 40 inches to the bottom. The range hood and exhaust system is required to comply with NFPA 96 and Chapter 3 of the Standard Mechanical Code. Note the requirement for fire suppression system. Response: The range top and hood will be deleted from the project scope. 6. Qualify that the main entrance is accessible. Response: When the entry door lockset is replaced with a lever handle type, the main entrance will be fully accessible. 7. Install a drinking fountain, or provide bottled water, or a sign that states you will provide water on request. Response: The owner provides and will provide bottled water receptacles. 8. No more than six (6) outlets are allowed on a circuit as per Ordinance #14385 Section 12-2 (c) in the City of Little Rock Code of Ordinances. GFCI protection is required for the electrical receptacles at the break room sink. Response: We will comply. 9. The permit fee will be calculated when the valuation of the project is submitted. Response: We concur. Hopefully, the above responses are found to be acceptable. If there are any questions, please contact me.
