PAGE 09/10 Little Rock Fire Department Fire Investigation Report Incident# 2005-013457 Exposure# 0 Report Narrative On July 24, 2006 Capt_ Dennis Free was dispatched to 1020 Rock Street to investigate a vacant commercial structure fire_ As I arrived on the scene I made contact with BC Adkins on the south side of the structure and was informed that the building was under remodel and that the electric power was still in service. Chief Adkins stated that the owner and an eye witness were waiting for me at the south west corner of 11th and Rock. Before making contact with the owner I made a site survey around the structure and noticed that the main body of the fire appread to in the south west corner of the structure and had already vented out the roof above the over head door area. I also noticed that a pickup truck and a mini van had burnt but had been extinguished before I arrived. As the fire was still being extinguished I spoke with Ryan Pertuit and Nova, both of 1620 Middle Row, Conway Arkansas, phone 358-2289_ Ryan stated that they were traveling 1-30 East and noticed an orange glow toward the downtown area and flowed 9th street west to the fire. Ryan stated that when he turned east onto 11th street he noticed the pickup truck, the light pole.on fire and had small flames above the south west corner of the structure and heavy smoke coming from the vents along the south wall and that the electric meter had blue sparks shooting out. Nova stated that she seen the same, but when Ryan ran to the house on the north side to wake the neighbors, the wall next r:o the pickup burst into flames and it looked like the whole place caught fire. I spoke with the owners Barbara and J.E.Core that were staying in a house sitting on the north west corner of the same property. J.E. stated that they had work inside the structure during the day on Sunday the 23RD of July and had left around 9:00 PM and had every thing turned off but a window air conditioner. Barbara stated that she had been awaken some tine after mid -night to take her dogs outside but did not noticed any thing wrong at that time. She said that she was awaken by the smell of something burning around 3:00 AM and when she looked out side she seen the power pole on fire and flames coming above the cover porch on the west. Barbara said that when she came around the southwest comer of the property she noticed the pickup and power pole on fire and flames coming out the roof area_ J.E. stated that when he came around the corner from the south west he noticed heavy flames coming out the south side of the structure and the pickup along with the power pole also on fire. I started my fire origin investigation at the south west section of the structure at the electric meter and found that there was internal damage to the meter box and the wiring from the pole had burnt lose from the meter. The breaker box was located along the east wall of the the old stock room and was remove by Capt. Free to the south side of the structure. The area of origin for the structure was located in the old storage room where the electric power enter the building and a short was Page 8 of 9 08/03/2006 09:18 5013714485 LR FIRE DEPT. I could not locate a source inside the structure that would cause the short. I then focused my investigation to the outside the structure which lead me to the Ford pickup. The engine compartment was totaly destroyed and the hood had been burnt away. The driver side front tire had burn from the engine side out. The fire spread toward the interior of the pickup and alone the driver side causing the power pole located just behind the driver door to catch fire. The fire extended up the the pole and cause the service wires to the meter loop on the structure to short out and send a power surge into the structure causing the structure to catch fire, The Ford F-150 Pickup, 1999, VIN #1 FTRX17L3XNB33020_ Also damage by the heat of the structure fire is a Chevy Astro Van, 1986, VIN # 1 G8DM15ZOGB166238. E2 laid 500" of 5" hose from 12th& Rock and used Deckgun until T1 setup. Pumped into T1 with 100" of 3" hose 2hrs_ Used booster line to overhaul. FM 2 investigated. Cause of the fire was a possible Electrical short at the breaker box. See FM 2 Capt. Free's for more info. Total Loss $200,000. Insurance Co. Brown & Brown_ 2 vehicles were damaged. 1999 Ford XLT P/u and a 1986 Chevy Astro Van Approx. $25,000.00 damage to both vehicies.Witness & Caller Ryan Pertuit advised that Electric Sox on the Southwest Side of the building was on fire and fire was coming out of the overhead next to it_ Smoke was also coming out of the vents along the roof, On 07/24/2006 at 03:08 dispatched to 1020 Rock ST.
