1. Provide the maximum number of children at any given time.
2. Provide the maximum number of caregivers at any given time. 3
3. Provide the hours of operation (times/days of the week).
4. Use survey to create a site plan depicting the following:

b. Outdoor play area.
5. Will there be a sign? if so, the sign must comply with Section 36-551 of the City's
Zoning Ordinance (signs permitted in residential one — and two-family zones). .�CON
