Dear Brian,
I am in receipt of you letter dated 7-15-13 and want to first apologize for violating any rules or regulations, it
was done out of ignorance and not willfully. Please accept my COA application per your request.
We are asking that the signage already placed on the building be considered for approval. The signage is
necessary as we are a walk in clinic and many of our clients will only visit us one time, therefore having posted
signage is critical for us. I even installed a 3`d sign recently facing Scott Street at the request of my neighbor, The
Fish Factory, who complained that many of our patrons are coming to their office believing it be First Choice.
The construction of the signs is aluminum with vinyl lettering, the dimensions of all 3 signs is 2ft x 8ft. Per your
instructions, in lieu of drawings I am submitting photos of the signs. As a reference our building is 10ft tall.
I have hired an abstract company to provide ne a list of property owners within 150ft of our property, I will be
following instructions and notifying them per certified mail.
I appreciate your professionalism in handling this matter and I am excited about being a new business resident
to the neighborhood. I certainly want to be a good neighbor and comply with all rules and regulations.
