Hi Marc,
The white and green certified mail receipt forms prove that you sent the mail, formalized as PS Form
3800. They look like the attached file called “USPS3800.” You will fill out each recipient’s name and
address. The Post Office will stamp each form, as shown in the attached “PS form 3800 examples” PDF.
