Submit a “variance/waiver request form” with the following variances:
Variance to allow reduced lot width for both lots.
Variance to allow reduced lot area for both lots.
Variance to allow increased lot width to depth ratio for both lots.
Submit completed “Information Sheet.”
Submit completed “Affidavit” (property owner authorization).
