From the limited information provided, it appears that Entergy has no facilities in conflict. If any facilities are found to
be in conflict and require relocation, customer will need to make arrangements with us to relocate those at his expense.

Approved engineering drawings for streetlight system have been received and approved. Applicant is
responsible to furnish as -built drawings once streetlights have been installed.

The Grantor does hereby certify that it has replatted the real estate in accordance
with the plat. The lands embraced in the plat shall be forever known as designated on the
plat, and every deed of conveyance for the Property shall use this designation.
The filing of this Bill of Assurance and plat for record in the Office of the Circuit
Clerk and the ex-officio Recorder of Pulaski County shall be valid and complete delivery
for the dedication of streets and easements shown on the plat, such easements restricted
to the specific purposes shown on the plat and in the width, length and location
designated on the plat, and the easements shall not be used for ingress and egress by the
public or other persons whomsoever and for the revocation of any and all streets and
easements shown on prior plats or replats.
All buildings constructed on the property shall be constructed no nearer to the

conformance with the Building Code and Zoning Ordinance of the City of Little Rock,

There shall be no changes to the Bill of Assurance without prior approval from
the City of Little Rock Planning Commission.
