Dear commission, We are hereby applying for a Special Permit for the new construction of a tiny
home on the lot of 4424 Lee Ave. specifically located in the rear yard. The new construction is to
replace an existing shed that is to be removed from the project site. Relating to the proposal,
please find attached the following: (list all documents, maps, reports, fees and other information
submitted with your application.)
As contractor of the proposed project, I hereby authorize, that Sara R. Baker has signed an
affidavit allowing me to proceed on her behalf for all permits and approvals related to this
property.
