BEFORE ME, the undersigned authority, personally appeared John David Ward, who, being first
duly sworn, deposes and states as follows:
1- 1, John David Ward, residing at 32 Kensington Dr, Conway, AR 72034, affirm that f have
been duly authorized by Sara Baker, the legal owner of the property located at 4424 Len
Ave. Little Rock, AR 72205 (hereinafter referred to as "the Homeowner"), to act on their
behalf for all matters related to the construction project at the aforementioned property.
2 1 am authorized to make decisions, enter into agreements: and take necessary actions to
manage and oversee the construction project on behalf of the Homeovmer.
3. 1 will act in good faith and in the best interests of the Homeowner while executing all,
responsibilities and decisions pertaining to the project.
4. This affidavit shall remain valid until revoked in writing by the Hornearmer or completion
of the construction project.
5. 1 affirm under penalty of perjury that the foregoing is true and correct
