(Completed Notice of Public Hearing Form - attached). Copy of the certified abstract list. (Please note the abstract list must come from a licensed title and/or abstract company). Certified mail receipts (The small green/white receipts reflecting the postmark stamp affixed at the post office. Please fill out the receipts to include the landowner’s address provided by the title search).   Finally, applicants are required to provide a photo to staff demonstrating the sign provided by this office was posted at the site.
