Provide notification of property owners located within 200-feet of the site, complete with the certified abstract list, notice form with affidavit executed and proof of mailing. 2. Provide on the proposed site plan the location of the existing septic system and fill lines servicing for the existing restaurant and the location of the proposed septic system and fill lines for the new building. Provide Health Department approval of the proposed septic systems. 3. Provide details of any proposed or existing signage (height/area/location) on the proposed site plan. 4. Provide the days and hours of operation of the proposed office development. 5. Provide the estimated number of employees of the proposed office development. 6. Provide the total square footage of the proposed office building on the proposed site plan. 7. Provide the maximum building height of the new building. Kanis Road and Ferndale Cutoff are classified on the Master Street Plan as principal arterials. Dedication of right-of-way 55-feet from centerline will be required. According to the survey, the existing restaurant is within 40-feet of the centerline. The proposed building apparently is outside of the future right-of-way line of 55-feet from centerline, but would be set back only 10-feet. This should be verified because sketch drawings and dimensions added to the survey do not match the original survey scale. 2. A 20-foot radial dedication of right-of-way is required at the intersection of the streets. 3. Provide design of the street conforming to the Master Street Plan. Construct one- half street improvement to the streets including 5-foot sidewalks with the planned development. Public Works would support a five year deferral. 4. Show all current and proposed driveway locations. New driveway locations are limited to 36-feet in width and should be located as far back from the intersection as practicable. Utilities and Fire Department/County Planning: Wastewater: Located outside the service boundary. No comment. The applicant must provide approval from the Arkansas Department of Health concerning the proposed septic system treatment. Enter : No comment received. Center -Point Energy: Approved as submitted. In addition to the nine -foot wide street buffer along Ferndale Cut-off, a nine -foot wide street buffer is required along Kanis Road. Additionally, a nine -foot wide land use buffer is required along the eastern and southern perimeters.
