Dear Mr. White:
The plans and specifications for the captioned project, dated September 26, 2008, prepared by Tim Daters, P.E., and submitted
to the Division of Engineering on September 29, 2008 have been reviewed and are hereby approved with the following
conditions.
1. The Division of Engineering relied upon the statements and representations made in the engineer's report,
plans, and specifications. In case any statement or representation in the aforementioned documents is found
to be incorrect, this Approval may be revoked.
2. There shall be no deviation from the plans and specifications unless revised plans and specifications have
been first submitted for review and written consent given,
3. The review and approval of the plans and specifications were for functional and sanitary features and in no
way constitute an analysis of the structural design.
4. If construction on this project is not started within one year of the date affixed hereto, this Letter of Approval
is void.
5. The engineer shall provide construction inspection services. Failure of the engineer or another party
approved in writing by this office to provide construction inspection may cause this approval to be revoked,

Little Rock Wastewater has reviewed the above referenced plans. They are approved as
submitted with one (1) copy being returned to you for your files with this letter. Approval is
being granted to the applicant to construct and install a sewer main extension. The applicant shall
be responsible for all costs associated with the installation of the sewer main extension in order
to obtain wastewater service from the Little Rock Wastewater (LRW). Please be advised that this
approval is based on the following standard and special conditions:
1. All significant changes in the plans for the proposed construction must be approved prior
to construction and all construction shall comply with the applicable requirements and
specifications of the Little Rock Wastewater,
2. Continuous project responsibility by the engineer is an express condition of this plan
approval. Reference is made to Part 4 of Section 00100 Standard Specifications For
Sewer Construction In The City of Little Rock.
3. The name of the Resident Inspector shall be furnished to LRW prior to the start of
construction. No construction may begin until this Approval Letter is acknowledged
by the Applicant (Owner) and returned to Little Rock Wastewater.
4. This approval letter shall be in force for a period of one (1) year. If construction of the
project has not begun within the one year period, LRW's approval of the project will
expire. No fees paid to the Little Rock Wastewater for this project shall be reimbursed
should the project approval expire.
5. Sanitary sewer lines shall be kept a minimwn horizontal distance of 5' from all
underground utilities (Including Storin Drainage) except water lines, which shall be kept
a minimum distance of 10' from sanitary sewer lines.
6. Connection of the proposed sewer main to the existing sewer main shall not be made
until the proposed sewer main has been tested, inspected, and accepted by the Little
Rock Wastewater. The applicant shall convey title to the sewer main extension
together with all necessary easements, free and clear of all liens and encumbrances,
by the appropriate documents prior to the connection.

7. This project is connecting to an existing Project (20425) that has not been accepted
by LRW at this time. No connection will be made to the existing manhole at the end
of Project 20425 until that project has been accepted in its entirety by LRW.
Ductile Iron Pipe shall be used on this project any time the cover to the existing or
proposed ground is less than thirty inches (30") or where clearance between two pipes is
less than eighteen inches (18") or where the utility deems it necessary (See Returned
Plans). It shall be the Engineer's and the Contractor's responsibility to check the depth of
cover and the clearance.
9. All Existing and Proposed Manholes shall be located outside of the top bank of all
existing or proposed ditches and shall be at such an elevation as not to allow rainwater
entry during periods of normal rainfall. Manholes with a difference in elevation of two
feet or greater shall be a Drop Manhole,
10. All sewer mains shall be constructed within easements meeting Utility Specifications
or in a public right of way. Minimum width of proposed easement shall be ten (10')
feet (or the depth of cover whichever is greater) and shall be equal distance either
side of the sewer main as constructed. No permanent foundation allowed within the
10' easement.
11. No sewer main may be constructed under or within ten feet of any existing detention
basin or any detention basin being constructed for this project.
12. Manhole steps are not allowed in LRW manholes. See 2007 Specifications
13. All buildings must be capable of being served by gravity sewer service. If minimum floor
elevations are required, it is the responsibility of the Developer to notify the builder or
Owner to assure gravity service is maintained. Individual pump stations will not be
accepted without LRW approval. This includes basement floor elevations. Plans show 6"
services to several of the buildings. All 6" service lines must run straight grade and
alignment between the building and the manhole they tie into.
14. This project will be administered using the Little Rock Wastewater 2007 Standard
S ecifications For Sewer Construction In The City Of Little Rock. Should you or
the Owner have any questions concerning specifications, please contact the Utility.
Engineer's Affidavit of Accuracy will be required to be submitted with Final
Paperwork for acceptance of this project into the LRW System .
15. Applicant agrees to obtain all necessary permits and approvals from all other governing
agencies including any applicable Improvement District prior to the start of construction.
APPLICANT SHOULD MAKE AN INDEPENDENT DETERMINATION OF the
existence, status or amount of any fees which may be due third parties, such as
Improvement Districts, which may be owed to the third party in addition to those fees due
to Little Rock Wastewater for sewer lines and or sewer facilities installation proposed by
applicant. The Little Rock Wastewater and Little Rock Sanitary Sewer Committee have
no such information of amounts due third parties, if any, and assume no responsibility for

furnishing such information to Applicant and expressly disclaim any such duty or liability
of any nature whatsoever arising out of Applicants failure to make that determination,
Special Conditions
1. Applicant agrees to pay the following charges prior to start of construction of the project:

2. Capacity Contribution Charge $0.00
3. This Project Will Not be eligible for reimbursement.
Approval of this project is conditional on the items mentioned above. It shall be the Engineer's
responsibility to notify the Applicant (Owner) and the Contractor of all the items mentioned
above. Failure to notify the Applicant of any of the items mentioned above does not relieve the
Applicant from constructing the project according to Little Rock Wastewater Specifications and
the terms of this approval letter,

Comment: The City of Little Rock reserves the right to require additional controls as needed. This

Dear Mr. Dale:
The design and drainage plans for the above referenced project are approved with the
following conditions and concerns:
1, Prepare a letter addressing street lights as required by Section 31-403 of the Little Rock Code.
Contact Traffic Engineering at 379-1813 (Steve Philpott) for more information.
2. A grading permit must be obtained prior to the initiation of work. Permits may be acquired from
the Public Works Department at 701 West Markham Street after approval of erosion control plan.
Contact Vince Floriani at 371-4817 with any questions,
3. The contractor is responsible for contacting Dow Currier at 371-4835 for inspection of all work in
the right-of-way.
4. At the completion and acceptance of the work in the right-of-way, the Contractor shall submit a
one (1) year maintenance bond for 50 percent of the total cost of improvements in the right-of-way
and easements. Also, provide as -built drawings of installed infrastructure.
5. The Contractor is responsible for contacting Traffic Engineering at 371-4452 to obtain barricade
permit prior to construction.
6. Contact Mel Hall, Public Works Civil Engineering, at 918-5217 for on -site clarification of grading
permit requirements prior to commencing work,
7. Tracking of mud is not permitted on city streets. The contractor is subject to fines and Stop Work
penalties for violation of this ordinance.
8. Any work involving one or more acres of disturbed area requires a State NPDES permit prior to
grading plan approval. Contact the Arkansas Department of Environmental Quality, NPDES
branch at 682-0744 for applications and information. about general permit HARR150000.
9. All erosion control features must be installed and maintained for duration of construction until
80% of the site is stabilized.
10. Currently a 5 ycar deferral for the half street for the improvements to 36"' Street or the
development of the adjacent property has been approved as conditions of this site.
] ] . Repair or replace any curb and gutter or sidewalk that is damaged in the public right-of-way prior
to occupancy.
12. On site striping and signage plans should be forwarded to Public Works, Traffic engineering for
approval with the site development.
