Whereas, Baptist Health, an Arkansas nonprofit corporation, with principal offices at
9601 I-630, Exit 7, Little Rock, Arkansas 72205 ("Grantor") is the sole owner of the following
described lands situated in Pulaski County, Arkansas, to wit:
PROPERTY DESCRIPTION
Part of the Northeast 1/4 of the Northwest 1/4 of Section 10, Township 1 North,
Range 13 West, Pulaski County, Arkansas being more particularly described as
follows:
Commencing at a found Cotton Picker Spindle in the Centerline of Kanis Road
and the Centerline of Jr. Deputy Road, also being the Northeast Corner of the
Northwest 1/a, , said Section 10, T-1-N, R 13-W; thence along the North line of
said Section 10, North 87' 35' 3 1 " West 379.81 feet to a point; thence South 02'
30' 06" West.40.93 feet to a Rebar found at the Northeast corner of Lot 2A Baptist
Health-Kans South Addition, said point also being on the South right of way line
of Kanis Road; thence continuing South 02' 30' 06" West along the east line of
said Lot 2A and 2B, Baptist Health-Kanis South Addition a distance of 518.63
feet to the Southeast corner of said Lot 2B, said point also being the POINT OF
BEGINNING; thence continuing South 02' 30' 06" West 380.75 feet; thence
North 87' 48' 35" West 248.49 feet to a point on the east right of way line of
Wilson Road; thence along said East right of way line North 02' 19' 12" East
380.75 feet to the Southwest corner of said Lot 2B; thence along the South line of
Lot 2B, South 8748' 35" East 250.81 feet to the Point of Beginning, containing
95,053 square feet, or 2.182 acres, more or less.
Whereas, said owner desires to plat the above property into two lots;
NOW, THEREFORE, the undersigned, for and in consideration of the benefits to
accrue to it and its successors and assigns, which benefits it acknowledges to be of value, has
caused to be made the Plat entitled "Lot 3A & 313, Baptist Health-Kanis South Addition"
showing a survey certified by B. Michael Watson, Registered Professional Land Surveyor, and

Certificate of Final Approval executed by the Little Rock Planning Commission, and showing
the bounds and dimensions of the property now being subdivided into lots (the 'Plat").
The lands in said Plat shall be forever known as Lots 3A and 3B, of the Baptist Health
Kanis South Addition to the City of Little Rock, Arkansas, and any and every deed of
conveyance of any lot in such addition describing the same by the number shown on said Plat
shall always be deemed a sufficient description thereof.
All buildings constructed on said lots shall be constructed no nearer to the street than the
building line shown on the plat, and all buildings shall be constructed in conformance with the
Building Code and Zoning Ordinance of the City of Little Rock, Arkansas.

ON THIS DAY before me, the undersigned authority, in and for said county and state,
personally appeared Larry Lazenby, Senior Vice President, of Baptist Health, an Arkansas
nonprofit corporation, and stated and acknowledged that he was duly authorized to execute the
foregoing instrument for and in the name and behalf of said corporation, and that he had signed,
executed and delivered the foregoing instrument, in his aforesaid capacity, for the consideration,
uses and purposes therein mentioned and set forth.

Re: Final Plat for Lot 3A &3B,Baptist Health - Kanis South Addition.
Dear Mr. Moore:
Please find attached 3 copies of the final plat for your review. A copy of the Bill of Assurance
is enclosed for your review. Please find attached the utility sign offs sheet. I will forward one
to copy to Mr. Dewayne Dixon for Public Works approval.
According to the Preliminary Plat there is a street directly south of Lot 3B. It is my
understanding the City of Little Rock has an option to buy the land shown as Lots 4, 5 & part
of 6 for a Community Center or Park adjacent to Police Substation and the street will not be
constructed.
