WHEREAS, said owner desires to plat the above property into a lot;
NOW, THEREFORE, the said Lot 1, Baptist Health-Kanis South Addition, hereinafter referred
to as the "Grantor", has caused said property to be surveyed by The Mehlburger Firm, Registered
Professional Engineers and Registered Professional Land Surveyor, and a plat thereofwhich in identified
by the title, "Lot 1, Baptist Health-Kanis South Addition", to the City of Little Rock,

officio Recorder of Pulaski County, Arkansas and being now of record in Plat Book _ at page _,
in the records of the said Circuit Clerk; therefore, the Grantor herein does make this Bill of Assurance.
The Grantor hereby certifies that it has laid off, platted and subdivided, the said lands
in accordance with said plat. The lands embraced in said plat shall be forever be known and
described as "Lot 1 Baptist Health-Kanis South Addition" to the City of Little Rock, Pulaski County,
Arkansas, and conveyance of said land by lot numbers shown on the plat shall be considered a complete
and proper description thereof.
All buildings constructed on said lots shall be constructed no nearer to the street than the
building line shown on the plat, and all buildings shall be constructed in conformance with the Building
Code and Zoning Ordinance of the City of Little Rock, Arkansas.
