KNOW ALL MEN BY THESE PRESENTS:
THAT, WHEREAS, LRFP Land L.L.C., an Arkansas limited liability company ("LRFP
Land"), is the sole owner of the following described lands known as Medical Plaza West and
lying in the County of Pulaski, State of Arkansas, to wit (the "Property"):
DESCRIPTION OF PROPERTY
See Exhibit "A"
and
WHEREAS, the Property was surveyed by White-Daters & Associates, Inc., Registered
Professional Land Surveyors ("White-Daters"), and a plat thereof made which is identified by the
title Medical Plaza West Addition and the date September 2 0 19 9 6;, nd by the
signature of the Circuit Clerk and ex officio Recorder of Pulaski County, Arkansas, in Plat Book
E, Page 743;.rand
WHEREAS, it is desirable that Lot 1 of the Property be replatted and the remainder of the
Property be platted into additional lots;
NOW, THEREFORE, WITNESSETH:
THAT LRFP Land has caused the Property to be re -surveyed by White-Daters and a plat
thereof made which is identified by the title Medical Plaza West Addition and the date June,
2001, and by the signature of the Circuit Clerk and ex officio Recorder of Pulaski County,
Arkansas, in Plat Book , Page (the "Plat"), and LRFP Land does hereby make
this Bill of Assurance, and

LRFP Land hereby certifies that it has laid off, platted and replatted and subdivided, and
does hereby lay off, plat and replat and subdivide the Property in accordance with the Plat. The
Property shall be forever known as Lots 1R, 2 and 3, Medical Plaza West, an Addition to the
City of Little Rock.
There are strips of ground shown and dimensioned on said plat marked "15' Sanitary
Sewer Easement," "Access and Utility Easement" and "Drainage Easement" reserved for the use
of public utilities and for drainage purposes, respectively, subject at all times to the proper
authorities and to delivery and dedication of the streets and easements shown on the Plat.
Hereafter, conveyance and description of the Property by lot number as shown on the Plat
shall be a proper and sufficient description thereof.
The lots in the Property may be sold by LRFP Land and shall be purchased by the buyers
thereof, subject to the following covenants, to wit:
No lot shall be re -subdivided into nor shall any structure be erected or placed on
any lot or building site that does not conform with the standards of the City of Little Rock
Planning Commission (the "Commission") either (i) in effect for the Property at that time, or (ii)
sought for the Property in a request to the Commission.
2. No obstruction shall be placed in the street or gutter. Curbs shall be broken at
driveways, and driveway aprons shall not extend past the face of the curb.
3. These covenants and restrictions are to run with the land and shall be binding on
all parties and all persons claiming under them for a period of thirty (30) days from the date these
covenants and restrictions are recorded, after which time said covenants and restrictions shall be
automatically extended for successive periods of ten (10) years, except that these covenants and

restrictions may be amended, canceled or supplemented by any owner of a lot (or portion
thereof) within the Property, if approved by the Commission.
4. The invalidation of any one of these covenants or restrictions by judgment or
court order shall in no wise affect any of the other provisions, which shall remain in full force
and effect.
The considerations set forth in items one through four are hereby approved by the
Commission. Any additional requirements, restrictions or statements are entered without review
or action of the Commission, and shall not be considered part of the Commission's requirements
for plat approval.
Reviewed only for inclusion of minimum standards
required by the City of Lite Bock subdivision regulations.
