Provide notification of the abutting property owners including the certified abstract list, notice form with affidavit executed and proof of mailing. The notice must be mailed no later than December 3, 2014. The Office of Planning and Development must receive the proof of notice no later than December 12, 2014. 2. The property must be annexed into the City limits of the City of Little Rock to receive City sewer service. 3. Will the subdivision maintain ownership of the floodway? 4. Are there any variances being requested to allow for reduced setbacks, (side or rear)? 5. Provide the names of owners of all unplatted tracts abutting the plat area and names of owners of platted tracts in excess of 2'/2 acres. 6. Provide the names of all owners of any landlocked parcels within or abutting the plat area. 7. Indicate the municipal boundaries which pass through or abut the plat area. 8. Provide a phasing plan if applicable. 9. Provide a draft Bill of Assurance. Variance/Waivers: 1. A variance from Section 31-232 to allow the creation of a pipe stem lot. 2. A variance from Section 31-2 to allow the creation of a pipe stem lot inconsistent with the minimum standards of the Subdivision Ordinance. Public Works Conditions: 1. Crystal Valley Road is classified on the Master Street Plan as a collector. A dedication of right-of-way 30 feet from centerline will be required. 2. With site development, provide the design of street conforming to the Master Street Plan. Construct one-half street improvement to Crystal Valley Road including 5-foot sidewalks with the planned development. Contact Nat Banihatti, Traffic Engineering, 501.379.1818 for additional information. 3. A grading permit in accordance with Section 29-186 (c) and (d) will be required prior to any land clearing or grading activities at the site. Other than residential subdivisions, site grading and drainage plans must be submitted and approved prior to the start of construction. Is the applicant requesting a variance to advance grade ITEM NO.: 2. S-1705-13 the lots with construction of the street? 4. Stormwater detention ordinance applies to this property. Show the proposed location for stormwater detention facilities on the plan. 5. If disturbed area is one (1) or more acres, obtain a NPDES stormwater permit from the Arkansas Department of Environmental Quality prior to the start of construction. 6. A special Grading Permit for Flood Hazard Areas will be required per Section 8-283 prior to construction. 7. In accordance with Section 31-176, floodway areas must be shown as floodway easements or be dedicated to the public. In addition, a 25 foot wide drainage and access easement is required adjacent to the floodway boundary. No fill dirt, structures, or fences are allowed to be installed within the floodway. 8. Streetlights are required by Section 31-403 of the Little Rock code. Provide plans for approval to Traffic Engineering. Streetlights must be installed prior to platting/certificate of occupancy. Contact Greg Simmons, Traffic Engineering, 501.379.1813 for more information. 9. Per the Master Street Plan, parking is restricted to one side of the street on a 24 foot wide street. Show on the plan now and on the final plat and bill of assurance, the area along the street where parking is restricted. 10. Contact Pulaski County Planning Department for conditions for developing property in the floodplain. City of Little Rock staff believes the finish floor elevations of the home should be at least 6 inches higher. 11. Provide a letter prepared by a registered engineer certifying the sight distance at the intersection(s) comply with 2004 AASHTO Green Book standards. The proposed street location has unsufficient sight distance for safe vehicle maneuvering. The driveway should be moved to the south near the south property line. 12.The proposed street name is a duplication. Street names and street naming conventions must be approved by Public Works. Contact Glenn Haley at 501. 371.4537. 13. Provide a Sketch Grading and Drainage Plan per Section 29-186 (e). Show the extent of grading on the west end of the cul-de-sac. 14. The 10 foot drainage easement is believed to be insufficient in width. A 100 year overflow path should be provided through the subdivision. Depending on the expected flows onto from the north, staff recommends the drainage path be placed within an undisturbed tract land. This does not pertain to the Haw Branch. 15.Alteration of the water course will require approval from the Little Rock District of the US Army Corps of Engineers prior to start of work for both the main creek channel and the north -south dry creek bed. 16.A minimum undisturbed strip 25 feet wide except for reasonable access shall be provided along each side of streams having a 10 year storm >150 cfs. The undisturbed strip should be measured from the top of the bank. Utilities and Fire Department/County Planning: Wastewater: Outside the service boundary. Must seek annexation to the City of Little Rock to receive City sewer service. Item # 2. ITEM NO.: 2, S-1705-B Entergy: Entergy does not object to this proposal. A 3-phase power line exists on the east side of Crystal Valley Road across from this development. A couple spans of wire cross the road to serve structures on the property which will need to be removed. Contact Entergy in advance to arrange for a service layout and to discuss easement requirements to feed all lots. Centerpoint Energy: No comment received. AT & T: No comment received. Central Arkansas Water: 1. All Central Arkansas Water requirements in effect at the time of request for water service must be met. 2. Due to the nature of this facility, installation of an approved reduced pressure zone backflow preventer assembly (RPZA) is required on the domestic water service. This assembly must be installed prior to the first point of use. Central Arkansas Water requires that upon installation of the RPZA, successful tests of the assembly must be completed by a Certified Assembly Tester licensed by the State of Arkansas and approved by Central Arkansas Water. The test results must be sent to Central Arkansas Water's Cross Connection Section within ten days of installation and annually thereafter. Contact the Cross Connection Section at 501.377,1226 if you would like to discuss backflow prevention requirements for this project. 3. This development will have minor impact on the existing water distribution system. Proposed water facilities will be sized to provide adequate pressure and fire protection. 4. The Little Rock Fire Department needs to evaluate this site to determine whether additional public and/or private fire hydrant(s) will be required. If additional fire hydrant(s) are required, they will be installed at the Developer's expense. 5. Please submit plans for water facilities and/or fire protection system to Central Arkansas Water for review. Plan revisions may be required after additional review. Contact Central Arkansas Water regarding procedures for installation of water facilities and/or fire service. Approval of plans by the Arkansas Department of Health Engineering Division and the Little Rock Fire Department is required. 6. A Capital Investment Charge based on the size of meter connection(s) will apply to this project in addition to normal charges. This fee will apply to all connections including metered connections off the private fire system. 7. If there are facilities that need to be adjusted and/or relocated, contact Central Arkansas Water. That work would be done at the expense of the developer. Fire Department: Maintain access, fire hydrants per code. Contact the Little Rock Fire Department for additional information. Item # 2. ITEM NO.: 2. S-1705-B County Planning: 1. Provide Bill of Assurance. 2. Show distances and bearings to second land corner, provide state plane coordinates. 3. Show adjacent ownership including subdivisions. 4. Provide development plan summary. 5. Provide approval from Volunteer Fire Department. 6. Provide street cross sections for Woodcreek Trace. 7. Provide profile for Woodcreek Trace. 8. Label all street centerlines. 9. Provide storm drainage plan. 10. Provide FEMA flood panel number on plat. Obtain flood development permit from Pulaski County Public Works. 11. Provide certificate of owner. 12. Woodcreek Trace must comply with Pulaski County Master Road Plan for a local road. Provide 60-foot right of way width. Provide 28-feet pavement width for Woodcreek Trace or 50-foot right of way and 27-feet of pavement width if curb and gutter is used. Dedicate additional right of way along Crystal Valley Road to equal one-half of 70-feet (35-feet) west from the Crystal Valley Road centerline. Cul-de- sac right of way radius must be 60-feet and the pavement radius must be 50-feet. Radius may be reduced to 50-feet if curb and gutter is used. 13. Provide full set of construction plans for Woodcreek Trace, including water and sewer plans. 14. Provide SWPPP for Pulaski County review. 15. Pay $33.00 review fee. 16. Provide certification that sight distance meet AASHTO requirements. Also, provide traffic study for this section of Crystal Valley Road. 17. Provide sidewalks on both sides of street if the development is within 1,320 feet of a school. CATA: Parks and Recreation: No comment received. Building Code: No comment.
