1. Provide notarized authorization affidavit (attached) I WILL SEND TO YOU AS SOON AS OWNER RETURNS THIS.
2. Provide description of exterior materials to be used (brick, siding, etc…) COMBINATION BRICK AND SIDING COMBINATION.
3. Provide site plan showing all proposed improvements to the property. SITE PLAN ATTACHED WITH THIS DOCUMENT.
