BILL OF ASSURANCE
of
FOUR OAKS LIVING CENTER
A Subdivision in Pulaski County, Arkansas
FOUR OAKS LIVING CENTER, LLC TO: THE PUBLIC
KNOW ALL MEN BY THESE PRESENTS:
THAT, WHEREAS, Four Oaks Living Center, LLC, an Arkansas
Limited Liability Company, (hereinafter called "Grantor"), is the
sole owner of the following described lands lying in the County of
Pulaski, State of Arkansas, to -wit:
Part of the SW 1/4, SW 1/4, Section 11, T-1-N, R-13-W,
Pulaski County, Arkansas, more particularly described as:
Starting at the Northwest corner of the SW 1/4, SW 1/4,
said Section 11; thence S 880 18' 46" E along the North
line of said SW 1/4, SW 1/4, 726.64 ft. to the point of
beginning; thence S 880 18' 46" E and continuing along
said North line, 548.43 ft. to a point on the West right-
of-way line of John Barrow Road; thence S O10 54' 14" W
along said West right-of-way 379.0 ft.; thence N 880 18'
46" W and parallel with the North line of said SW 1/4, SW
1/4, 547.0 ft.; thence N 10 41' 14" E and perpendicular
to said North line, 379.0 ft. to the point of beginning,
containing 4.77 acres more or less.
NOW, THEREFORE, WITNESSETH:
THAT the said Grantor has caused said tract of land to be
surveyed by White-Daters & Associates, Inc., Registered
Professional Engineers by Joe White, Registered Land Surveyor No.
534, Arkansas, and a plat thereof made which is identified by the
title Final Plat Four Oaks Living Center and the date March 7,
1996, and by the signature of the Circuit Clerk and Ex-Officio
Recorder of Pulaski County, Arkansas, in Plat Book Page
and the grantor does hereby make this Bill of Assurance.
AND, the grantor hereby certifies that it has laid off,
platted and subdivided, and does hereby lay off, plat and subdivide
said real estate in accordance with said plat. The lands embraced
in said plat shall be forever known as Four Oaks Living Center.
The grantor hereby dedicates to the public forever an easement
right of way on and over the streets as shown by said plat, to be
used as public streets.
There are strips of ground shown and dimensions on said plat
marked "Utility Easement" and "Drainage Easement" reserved for the
use of public utilities, and for drainage purposes respectively,
subject at all times to the proper authorities and to the easements
filed with the Circuit Clerk and Ex-Officio Recorder of Pulaski
County, Arkansas.
The filing of this Bill of Assurance and plat for record in
the Office of the Circuit Clerk and Ex-Officio Recorder of Pulaski
County shall be a valid and complete delivery and dedication of the
streets and easements shown on the said plat.
Hereafter, conveyance and description of any of said lands by
lot number as shown on said plat, shall be a proper and sufficient
description thereof.
IN WITNESS WHEREOF, Richard B. Griffin II, Manager of Four
Oaks Living Center, LLC hereby sets his hand this *'I( —day of

the State of Arkansas and that said instrument was signed in behalf
of said company by its Secretary and acknowledges this
instrument to the free act and deed of said company.
In Testimony Whereof, I have hereunto set my hand and affixed
my official seal on the date above written.
