following conditions_
1. The review and a eptance of the information submitted (including project description. layout.
vicinity map, and soils information) were F(�r functional and sanitary features and in no way
constitute an analysis of 1€-1e strut#oral design.
2- If construction on this project is not started within orle year of the date affixed hereto- this Letter of
Acceptance is void.
3. Construction inspection for this project shall be the responsibility of Mr. Bodie Drake
DR#00926500 PSC 476 or other DR registered with the Arkansas 1D2paament of Health.
4. Ail conditions of the AD letter dated February 16. 2023, from Ryan Christensen, R- S . Assistant
Environmental Manager, shell be adhered to-
5. Please contact the Local Counly Health Unit for individual sewage disposal permit applications.
