The information submitted for the project referenced above, received on February 10, 2023 through March 14, 2023,
has been reviewed and is hereby accepted with the following comments:
1. The review and acceptance of the information submitted (including project description, layout, vicinity map,
and soils information) were for functional and sanitary features and in no way constitute an analysis of the
structural design.
2. If construction on this project is not started within one year of the date affixed hereto, this Letter of Acceptance
is void.
3. Construction inspection for this project shall be the responsibility of Mr. Bodie Drake DR#0926500 PSC #78
or other DR registered with the Arkansas Department of Health.
4. All conditions of ADH letter dated February 16, 2023, from Ryan Christensen, R.S. Assistant Environmental
Manager, shall be adhered to.
5. Please contact the Local County Health Unit for individual sewage disposal permit applications.
When submitting correspondence pertaining to this project, please include the ADH reference number 127869.
