Grantors: Blaine A. Burgess and Cindra K. Burgess
To: The Public
Whereas BLAINE A. BURGESS and CINDRA K. BURGESS, husband and wife (collectively,
the "Grantors"), are the sole owners of the real property described as Lot 3, Block 17, The
Villages of Wellington, an Addition to the City of Little Rock, Pulaski County, Arkansas as
shown on the plat of record in Plat Book G, Page 680 in the records of Pulaski County,
Arkansas, and Grantors desire to replat said lot to modify the front building setback line
currently applicable thereto.
NOW, THEREFORE, WITNESSETH:
That Grantors have caused the said tract of land to be surveyed by Daniel W. Havner of White
Daters & Associates, Inc., Registered Land Surveyor No. 1542, and a plat thereof made which is
identified by the title Plat of Lot 3R, Block 17, The Village of Wellington, An Addition to the
City of Little Rock, Arkansas and the date ,- - -�1,14 - -.a I I , 2019 and by the signatures of
the said surveyor and the said Grantors, and bears a Certificate of Final Approval executed by
the Little Rock Planning Commission, and is on record in the Office of the CircuitCCk and ex-
officio Recorder of Pulaski County, Arkansas, as Instrument No.
(the "Plat") and the Grantors do hereby make this Bill of Assurance. L3 S 9 0 3 D- 19
The Grantors do hereby certify that they have replatted said real estate as Lot 3R, Block 17, The
Villages of Wellington, in accordance with said Plat. The lands embraced in said Plat shall be
forever known as designated on said Plat; and every deed of conveyance for said property shall
use this designation.
The filing of this Bill of Assurance and Plat for record in the office of the Circuit Clerk and ex-
officio Recorder of Pulaski County shall be a valid and complete delivery and dedication of the
public easements and public rights of way as shown on the said Plat. This Bill of Assurance
shall be in full force and effect in perpetuity unless all of the record owners of the land subject to
this Bill of Assurance record a written amendment or termination of this instrument.

All buildings constructed on said lot shall be constructed no nearer to the streets than the
building lines shown on the Plat, and all buildings shall be constructed in conformance with the
Building Code and Zoning Ordinance of the City of Little Rock, Arkansas.
This Bill of Assurance supplements the Plat and Bill of Assurance recorded in the Office of the
Circuit Clerk and ex-officio Recorder of Pulaski County, Arkansas, as Instrument No.
2003054610, which remains in full effect with respect to the herein described property, subject to
the modifications set forth in this Bill of Assurance and/or as shown on the Plat. In addition, the
Declaration of Covenants and Restrictions recorded in the Office of the Circuit Clerk and ex-
ofcio Recorder of Pulaski County, Arkansas, as Instrument No. 95-47743, as amended and/or
restated from time to time, shall also remain in effect with respect to the herein described
property, subject to the modifications set forth in this Bill of Assurance and/or as shown on the
Plat.
