Beyond Boundaries Supportive Living, LLC, is to understand every client's path as they take
the proper steps to create independence and community productivity. We will focus on their
strengths while paying attention to their needs. We understand that each client's path will
be different as they work toward each goal in their own unique way. We are committed to
the wellness of each client through prevention, intervention, treatment, and education.
Beyond Boundaries Supportive Living, LLC provide services in a home equivalent
environment for their clients, while ensuring that they receive supportive health and social
services. This service will help maintain their individuality, independence, privacy, health,
and safety, while encouraging self -direction, and decision -making skills. These services will
be provided based on each client's care plan, which will always be assisted by Beyond
Boundaries Supportive Living, LLC staff. Beyond Boundaries Supportive Living, LLC staff will
strive to connect the client to their community, in effort to live to their full potential
comfortably. Supportive services included but are not limited to:
• Assistance in preparing meals
• Mobility
• Bathing
• Dressing
• Hygiene
• Medication
Beyond Boundaries Supportive Living, LLC case management services will strive to improve
Arkansans level of care, regarding mental health, disabilities, etc... This service will link
necessary providers to clients' unique needs, while coordinating community -based services
to meet their individualized care needs. Costs of services will be monitored to prevent
over/under utilization for repeat services for Beyond Boundaries Supportive Living, LLC's
targeted population.
All services will be in accordance with Provider -Led Arkansas Shared Savings Entity (PASSE)
Model of Care.
We also provide specialized medical supplies including durable and non -durable medical
equipment not available under the Arkansas Medicaid State Plan that will enable Beyond
Boundaries Supportive Living, LLC clients to overcome specific limitations with specialized
medical supplies. In addition, necessary medical supplies that is not available under the
Arkansas Medicaid State Plan will be requested according to clients' needs to overcome
their specific limitations with necessary medical supplies. Case management services will
coordinate acquiring medical supplies with Durable Medical Equipment (DME) providers.
Some medical supplies, but not limited to:
• Under -pads
• Communication devices
• Specialized rehabilitative equipment
• Wheelchairs
• Wheelchair seating systems
• Orthotic appliances
Beyond Boundaries Supportive Living, LLC, functions to make sound decisions based on our
core values: Compassion, Quality -Consciousness, Client -Focused Care, Dedication,
Innovation, and always putting the client first.

client(s) with Beyond Boundaries going forward.
