Your applicaƟon will be heard at the December 7th public hearing following the receipt of the below submiƩals in red. The hearing will start at 4:00pm at the Willie Hinton Neighborhood Resource Center at 3805 W. 12th Street. Please follow instrucƟons exactly for public noƟficaƟon requirements as outline in the COA applicaƟon document. I am sƟll waiƟng on the completed COA applicaƟon form, page 4 of the applicaƟon packet, and the notarized authorizaƟon of representaƟon, page 6. I’ve aƩached the COA applicaƟon here. Complete this and email it back to me, including the notarized authorizaƟon of representaƟon if the owner has a representaƟve. This should be for the person who plans on aƩending the public hearing as an agent on behalf of the owner. Please make sure to read through all of the instruc Ɵons in this applicaƟon packet especially for public noƟce requirements. You are welcome to stop by our office to pay the filing fee. At that Ɵme, you can pick up the required public noƟce signs provided by our office. This applicaƟon requires a public hearing and cannot be reviewed administraƟvely by staff. If you have any quesƟons regarding the public noƟce requirements, I’m happy to walk through it with you over the phone or in person when you come to pay the filing fee.
