Deaf Ms. James:
The property at 7618 Colonel Glenn Road has two small buildings. The applicant or related
parties have owned the property since the early 1980's. The southernmost building (fronting on
Colonel Glenn Road) has been used as a beauty salon during that time. Recently the tenant in the
beauty salon changed. Planning staff called to the owner's attention that the property, which is
zoned I-2 (light industrial), requires a Conditional Use Permit to be operated as a beauty salon.
For your consideration, I am enclosing the following documents, all of which, together with this
cover letter, are the applicant's application for a Conditional Use permit to use the southernmost
building as a beauty salon (the "Application"):
1. Application for Conditional Use Permit
2. Eighteen (18) copies of an ALTA survey prepared by White-Daters & Associates, Inc. dated
February 3, 2017, which Applicant is also submitting as its site plan ("Survey") or "Site
Plan");
3. Existing plat for the Douglass Addition dated February 23, 1962, and recorded in Plat Book
10, Page 96, records of Pulaski County, Arkansas (the "Plat").
4. Bill of Assurance dated April 24, 1962, and recorded on May 11, 1962, in Book 796, Page
233, records of Pulaski County, Arkansas (the "Bill of Assurance").
5. Affidavit of R. W. Butler Family Limited Partnership, authorizing Andrew V. Francis to act
as its agent.
6. Conditional Use Permit Street Right -Of -Way Dedication Agreement; and

Dear Dana:
One March 8, 2017, at the Subdivision Committee meeting on the above -referenced item, Planning
Staff requested additional information from the applicant. This is the applicant's response.
1. The typical number of employees will be two (2) stylists.
2. The hours of operation will be 9:00 am to 5:00 pm, Monday through Saturday.
3. The existing signage is a 3' by 1' barber pole. Any additional signage will comply with the I-
2 standards.
4. There will be a four (4) yard dumpster fully enclosed by opaque fencing on the right-hand
(eastern) side of the circular drive behind the buildings. The approximate dimension of the
fending will be ten (10) feet by six (6) feet, as shown on the attached drawing.
I previously sent proof of notice and proof of posting of the sign to Donna James. I will copy you on
those transmissions by separate e-mail.

Please find below Entergy's comments related to the items received from the Department of Planning
and Development this week. The request letter said to have the comments back to you by March 6th,
2017.
Village at the Gateway Tower use permit —12506 Vimy Ridge Rd.: Z-9204
Entergy does not object to this proposal. An existing three phase, power line exists to the east of
this location along Vimy Ridge Road. It is not in conflict with the proposed development.
Contact Entergy in advance to discuss future service requirements, new facilities locations and
adjustments to existing facilities (if any) as this project proceeds.

Entergy does not object to this proposal. Service is already being provided to this structure.

There are existing power lines adjoining, or crossing these properties in multiple locations. The
easements and ROW for these lines cannot be abandoned as they are being used to serve existing
customers in the area and need to be maintained for future use. Entergy will need to retain its
easements and 24 hour, vehicular access to its existing facilities for maintenance reasons.
Below is a screen shot from our GIS facilities database with the approximate boundaries of the
proposed property abandonment as previously communicated regarding this project. Contact
Entergy in advance to discuss any changes to location of any of these existing facilities as this
proposal proceeds.
Stanley Duplexes Conditional Use Permit — NW corner 401h and Gilman: Z-9199
Entergy does not object to this proposal. A single phase, overhead power line exists along the
south side of 40th Street at this proposed building location. There do not appear to be any
