1. How many residents will be at EACH location?
a. Rivercrest Drive — 5 residents

2. How many staff will be at EACH location?
Staff presence if provided in rotating 8-12 hour shifts. At least 1 staff, but
no more than 2 staff will be present at each location if any resident is
present.
Describe the services that will be provided to residents at EACH location.
a. Supported Living services through CES Waiver will be provided at each
location.
4. Will any of the residence drive?
a. If a resident is a licensed driver and has access to a vehicle, they are
permitted to drive.
Provide a revised site plan to show the parking where staff, residents or
visitors will park for EACH location.

6. Provide square footage of EACH residence.

7. Provide information about the bedroom accommodations including square
footage of each bedroom and the number of residents per bedroom FOR EACH
RESIDENCE.
a. Each residence is set up to accommodate 1 resident per bedroom providing
more than the 50 square ft of personal living space required by the CES
Waiver standards by which we are regulated.
