GENERAL NOTES
1. THE GENERAL CONTRACTOR SHALL TAKE ALL PRECAUTIONS NECESSARY TO
AVOID PROPERTY DAMAGE TO ADJACENT PROPERTIES DURING THE
CONSTRUCTION PHASES OF THIS PROJECT. THE CONTRACTOR WILL BE HELD
SOLELY RESPONSIBLE FOR ANY DAMAGES TO THE ADJACENT PROPERTIES
OCCURRING DURING THE CONSTRUCTION PHASES OF THIS PROJECT.
2. WARRANT`A/DISCLAIMER
THE DESIGNS REPRESENTED IN THESE PLANS ARE IN ACCORDANCE WITH
ESTABLISHED PRACTICES OF CIVIL ENGINEERING FOR THE DESIGN
FUNCTIONS AND USES INTENDED AT THIS TIME. HOWEVER, NEITHER
THOMAS ENGINEERING COMPANY, INC., NOR ITS PERSONNEL CAN OR DO
WARRANTY THESE DESIGNS OR PLANS AS CONSTRUCTED EXCEPT IN THE
SPECIFIC CASES WHERE THOMAS ENGINEERING COMPANY PERSONNEL
INSPECT AND CONTROL THE PHYSICAL CONSTRUCTION ON A
CONTEMPORARY BASIS AT THE SITE.
3. SAFETY NOTICE TO CONTRACTOR
IN ACCORDANCE WITH GENERALLY ACCEPTED CONSTRUCTION PRACTICES,
THE CONTRACTOR WILL BE SOLELY AND COMPLETELY RESPONSIBLE FOR
CONDITIONS OF THE JOB SITE, INCLUDING SAFETY OF ALL PERSONS AND
PROPERTY DURING PERFORMANCE OF THE WORK. THIS REQUIREMENT WILL
APPLY CONTINUOUSLY AND NOT BE LIMITED TO NORMAL WORKING HOURS.
THE DUTY OF THE ENGINEER OR OWNER TO CONDUCT CONSTRUCTION
REVIEW OF THE CONTRACTOR'S PERFORMANCE IS NOT INTENDED TO
INCLUDE REVIEW OF THE ADEQUACY OF THE CONTRACTOR'S SAFETY
MEASURES, IN, ON I OR NEAR THE CONSTRUCTION SITE.
4. ALL SLOPES TO B GRADED TO 3:1 OR FLATTER UNLESS OTHERWISE
INDICATED ON THIS SHEET.
5. REINFORCED CONCRETE PIPE (RCP) SHALL BE CLASS III.
6. ALL. STORM DRAIN INES AND UTILITY LINES UNDER THE PAVEMENT SHALL BE BACK FILLED
WITH CRUSHED ST E.
7. ENGINEER'S NOTICII TO CONTRACTOR
THE CONTRACTOR IS SPECIFICALLY CAUTIONED THAT THE LOCATION
AND/OR ELEVATION OF EXISTING UTILITIES AS SHOWN ON THESE PLANS IS
BASED ON RECORDS OF THE VARIOUS UTILITY COMPANIES AND, WHERE
POSSIBLE, MEASUREMENTS TAKEN IN THE FIELD. THE INFORMATION IS NOT
TO BE RELIED ON AS BEING EXACT OR COMPLETE. THE CONTRACTOR MUST
CALL THE APPROPRIATE UTILITY COMPANY AT LEAST 48 HOURS BEFORE ANY
EXCAVATION TO REQUEST EXACT FIELD LOCATIONS OF UTILITIES. IT SHALL
BE THE RESPONSIBILITY OF THE CONTRACTOR TO RELOCATE ALL EXISTING
UTILITIES WHICH CONFLICT WITH THE PROPOSED IMPROVEMENTS SHOWN
ON THE PLANS.
8. FWISHED GRADE CONTOURS ARE INDICATED ALONG TOP OF COMPLETED
STRUCTURES, TOP OF PAVEMENT AND GUTTER LINE OF CURB, UNLESS
OTHERWISE SHOWN. FOR ROUGH GRADING, CONTRACTOR SHALL ALLOW
FOR DEPTHS OF TOPSOIL AND CONCRETE STRUCTURES. FOR FINISH
GRADING, CON ACTOR SHALL INSTALL TOPSOIL AND CONCRETE
STRUCTURES TO NISHED GRADE AS INDICATED ON THIS SHEET.
9. GRADE TO DRAIN AWAY FROM BUILDING AT MINIMUM 1% SLOPE.
10. THE GRADING CONTRACTOR SHALL INSTALL STRAW BALES OR SILT FENCE AROUND ALL
DRAINAGE BOXES DURING CONSTRUCTION TO CONTROL EROSION AND
S!_TATION OF THE SITE.
11. ALL DIMENSIONS RE SHOWN TO THE BACK OF CURB.
12. CONCRETE RESIDE TIAL DRIVES SHALL BE POURED DURING BUILDING CONSTRUCTION AND
ARE NOT PART 0 THIS CONTRACT.

CIVIL ENGINEERING COMMENTS:
1.A GRADING PERMIT MUST BE OBTAINED PRIOR TO THE INITIATION OF WORK. PERMITS
MAY BE ACOUIRED FROM THE PUBLIC WORKS DEPARTMENT AT 701 WEST MARKHAM
STREET AFTER APPROVAL OF EROSION CONTROL PLAN. CONTACT VINCE FLORIANI AT
371-4817 WITH AN QUESTIONS.
2.HALILING OF FILL MATERIAL ON OR OFFSITE OVER MUNICIPAL STREETS AND ROADS
RECUIRES APPROVAL PRIOR TO A GRADING PERMIT BEING ISSUED. CONTACT PUBLIC
WORKS TRAFFIC ENGINEERING AT 621 S. BROADWAY, 379-1805 (TRAVIS HERBNER) FOR
MORE INFORMATION,
3.THE CONTRACTOR IS RESPONSIBLE FOR CONTACTING MEL HALL (501-590-7465) FOR
INSPECTION OF ALL WORK IN THE RIGHT-OF-WAY.
4.AT THE COMPLETION AND ACCEPTANCE OF THE WORK IN THE RIGHT-OF-WAY, THE
CONTRACTOR SHALL SUBMIT A ONE (1) YEAR MAINTENANCE BOND FOR 50 PERCENT OF
THE TOTAL COST CF IMPROVEMENTS IN THE RIGHT-OF-WAY AND EASEMENTS. ALSO,
PROVIDE AS -BUILT DRAWINGS OF INSTALLED INFRASTRUCTURE.
5.CONTACT MEL HALL, PUBLIC WORKS CIVIL ENGINEERING, AT 918-7465 FOR ON -SITE
CLARIFICATION OF GRADING PERMIT REQUIREMENTS PRIOR TO COMMENCING WORK.
&TRACKING OF MUD IS NOT PERMITTED ON CITY STREETS. THE CONTRACTOR IS SUBJECT
TO FINES AND STOD WORK PENALTIES FOR VIOLATION OF THIS ORDINANCE.
7.AN'! WORK INVOLVING ONE OR MORE ACRES OF DISTURBED AREA REQUIRES A STATE
NPIIES PERMIT PRI R TO GRADING PLAN APPROVAL. CONTACT THE ARKANSAS
DEPARTMENT OF ENVIRONMENTAL QUALITY, NPDES BRANCH AT 682-0744 FOR
APPLICATIONS AND INFORMATION ABOUT GENERAL PERMIT #ARR150000.
BALL. EROSION CONTROL FEATURES MUST BE INSTALLED AND MAINTAINED UNTIL THE SITE
IS STABILIZED.
9. ALL DISTURBED AREAS MUST BE VEGETATED WITHIN 21 DAYS AFTER CONSTRUCTION IS
