1. The plat of survey prepared by me was actually made by an
on the ground survey on 12/22/08 and to the best of my
knowledge, information and belief, all information hereon is true
and accurately shown;
2. Except as shown, the size, location and type of buildings and
improvements are as shown in field;
3. Surveyor made a phone call to the City of Little Rock Planning
& Development to obtain zoning and setback information. The
setbacks are as shown on survey.
4_ All monuments shown actually exist, and the location, size and
type of materials are correctly shown;
5. Surveyor cannot certify that all utility services required for the
operation of the property either enter the property through
adjoining public streets or the survey shows the point of entry and
location of any utilities that pass through or are on adjoining
private land;
6. By graphic plotting only, this parcel described hereon does not
