Provide notification of abutting property owners of the site, complete with the certified abstract list, notice form with affidavit executed and proof of mailing. The notice must be mailed no later than December 3, 2008. The Office of Planning and Development must receive the proof of notice no later than December 12, 2008. 2. Provide the days and hours of use of the facility. 3. Provide the seating capacity of the church. Provide the membership enrollment and the maximum enrollment anticipated at this location. 4. Provide details of any proposed signage including ground mounted and building signage. Provide the location of ground mounted signage, the total height and total area. Will electronic signage be requested at this site? Provide the percentage of the facade proposed for signage on the building. 5. Will a dumpster be utilized on this site. If so provide the location of the dumpster facility along with a note if the hours of service are to be limited to daylight hours. 6. The site contains a limited number of parking spaces in front of the building. If the rear parking area is to be used as the required parking the area must be opened -up and the parking area must be resurfaced (all parking is to be a hard surface material). Variance/Waivers'. None requested. Public Works Conditions- 1 . Repair or replace any curb and gutter or sidewalk that is damaged in the public right- of-way prior to occupancy. 2. Baseline Road is classified on the Master Street Plan as a minor arterial. A dedication of right-of-way 45 feet from centerline will be required. Staff cannot determine the existing right of way due to the submitted plan is not prepared to scale. 3. Due to the proposed use of the property, the Master Street Plan specifies that Distribution Drive for the frontage of this property must meet commercial street standards. On December 2, 2008, the applicant requested this item be withdrawn from consideration without prejudice. Staff is supportive of the withdrawal request. Central Arkansas Water: meter(s) are needed. Contact Central Arkansas Water if larger and/or additional Fire Department: Approved as submitted. County Planning: No comment. CATA: The site is not located on a dedicated CATA Bus Route. Planning Division: This request is located in the Geyer Springs West Planning District. The Land Use Plan shows Light Industrial for this property. The applicant has applied for a rezoning from 1-2 to Planned Industrial Development to add a church as an allowable use. The request does not require a change to the Land Use Plan. Master Street Plan: Baseline Road is a Principal Arterial. The primary function of a Principal Arterial is to serve through traffic and to connect major traffic generators or activity centers within urbanized areas. Entrances and exits should be limited to minimize negative effects of traffic and pedestrians on Baseline Road since it is a Principal Arterial. The street may require dedication of right-of-way and may require street improvements for entrances and exits to the site. Bicycle Plan: There are no bike routes in the immediate vicinity. Neighborhood Action Plan: This area is covered by the Chicot West 1-30 South Neighborhood Plan. The Economic Development Goal states: "Provide a mixed commercial/residential environment that will promote the safety, attractiveness, and value of the area while creating a competitive and adaptable economic climate that encourages investment and diversity of employment opportunities." Landscape: 1. The site plan must comply with the City's minimal landscape and buffer ordinance requirements. 2. If the rehabilitation costs exceed fifty percent (50%) of the replacement cost of the building, then the parking lot and associated landscaping must be in full compliancy with both the buffer ordinance and the landscape ordinance. 3. All the landscaping, fencing, and dumpster enclosures should be in good condition or replaced in conjunction with this application. Item # 11.
