Development Staff Comments:
Provide Survey of property.
Provide total square footage of the residence.
Provide additional information on the people who will be residing in the family care facility (recovering substance abusers, disabled persons, homeless persons, etc….)
Provide detailed information as what types of services that will be provided to the residents.
Will there be any Staff on site?
Will residents drive/own their own vehicle?
If any residents are going to own vehicles or if staff will be reporting to the residence, please provide a parking plan.
