Dear Sir/Madame: Thank you very much for your assistance regarding the matter referenced above. Enclosed for your review are the following items: • Wireless Communications Facility application with legal description and site photo attached • $100 application fee • ERP Statement • (4) copies of site plan The proposed installation is a co -location on an existing tower that sits on a 40 acre parcel. The proposed un -manned facility will be located inside the existing tower compound and will not compromise setback and/or the characteristics of the surrounding area. The proposed installation includes the following: (1) UHF antenna located on the exiting 1,266' tower at 1,037; (1) 12 x 20 x 10 unmanned, pre -fabricated equipment shelter; (1) 12 x 8 concrete pad that will house a proposed generator; (1) 6 x 10 concrete pad that will house a proposed heat exchanger; (2) Ku dishes (approximately 6' in diameter located on the existing equipment facility; (2) GPS antennas (approximately 6" in height); and ice bridge and cabling that run from the equipment to the tower and proposed antennas.
