At that meeting, you were advised that City of Faith correctional facility had until March 4, 2001 to come into compliance with the provisions of Section 36-107 (13) of the Code of Ordinances of the City of Little Rock. A copy of that section is attached. It was determined that City of Faith would hold two (2) public hearings prior to the Planning Commission hearing. All property owners or persons operating a business within 500 feet of the facility's property line are to be notified of the public hearings, in accordance with the procedures outlined in Section 36-107 (13) (a). You must provide proof of the public hearings when you file the conditional use permit application. It was further determined that an application for a conditional use permit would be filed with our office no later than December 26, 2000, to be placed on the Planning Commission's February 8, 2001 agenda. You received, at the November 30 meeting, a conditional use permit application. It is my understanding that you have scheduled the public hearings for January 9, 2001 at the Northwest Police Precinct and January 29, 2001 at the Adult Center. I've asked an abstract company for the 500 ft. search and was told that I can't have the results before next Monday. Apparently that's not an instant, on your computer -type process. As soon as this happens I plan to get the certified letters out. I feel like time isn't on my side and, like I said, I'm at the mercy of everyone else's time table. The date of the first meeting had to be changed to January 9th due to the fact that Vision Little Rock meetings are held at the NW precinct every Monday. I was made aware later that this is also board meeting night .... let me know if another change has to be made before the letters and the advertisements go out. Thank you for your time and help. I'm having a Bureau of Prisons inspection this week (Wednesday). After this is over I'll have more time to focus on the subject at hand. How- ever, no conditional use authorized by the plan- ning commission shall be subsequently applied for a variance to the board of -zoning adjustment. Amendments or changes as to the use authorized by the conditional use process must follow the same process as the original conditional use ap- plication. Required permits must be obtained within the allotted period. Failure to obtain permits will result in notice of termination from city staff. I have discussed this problem with my management staff and have developed a solution to the problem. We request that you notify me, or Supervisory Deputy Jim Hall, when you notify the Bureau of Prisons Community Correction Manager that one of your clients has violated facility rules, or Bureau of Prisons regulations, and request the client be remanded to the custody of the Bureau of Prisons as a program failure. By providing us with advance knowledge that a program failure notification may be forthcoming from the Community Corrections Manager we can prepare to receive and transport your client to the designated secure facility. Keep in mind that, with exception of emergency situations, we will not pick up the client prior to receiving authorization from the Community Correction Manager. By establishing this notification procedure the effectiveness of both of our operations will increase. I am pleased that our open and honest discussion of a mutual concern has resulted in a solution that will increase the effectiveness of custodial and security responsibilities. Mr. Hambrick telephoned the facility at 6:30 am on November 4, 1998, and stated he was leaving for work. When he failed to telephone the facility later in the morning, your staff went to the job site and spoke with his supervisor and made two visits to his residence to see if he was at home. In addition, in your efforts to locate Mr. Hambrick, you made calls to his family members and checked with local hospitals and jails. You also kept this office informed of the situation and notified local law enforcement that Mr. Hambrick was not at his assigned location. City of Faith made every effort to assist Mr. Hambrick in a successful transition back to the community. Unfortunately, he relapsed to alcohol abuse and erroneously believed he could convince you and your staff that he was at work. The efforts by you and your staff, as well as the timely reporting of this incident, resulted in Mr. Hambrick's being arrested and placed in secure custody in a matter of four hours. The effective date for contract J200c-292, was October 1, 1996, and performance began November 30, 1996. This contract is currently operating in the sixth month of Option Year #2. The contract quota is 48. For the past six months, September 1, 1999 through February 29, 2000, City of Faith, Little Rock, utilized 11,035 regular inmate days and 2,127 home confinement days. The average daily population for the past six months has been 60.6 in-house and 11.7 home confinement residents. On the first day of the monitoring, there were 84 federal offenders under the supervision of facility staff. Special attention was given to facility maintenance, sanitation, case management practices, and resident accountability. The Director, Community Corrections Manager, and Contract Oversight Specialist discussed home confinement and addressed specific issues involving several residents. A fire drill was conducted. Three staff were individually interviewed, and nine staff members were finger -printed. Staff morale seemed excellent. Six residents were interviewed and seven others responded to the reviewers via a written survey. All residents interviewed and surveyed provided positive feedback about City of Faith and indicated that the personnel at City of Faith, Little Rock, were helpful and professional. We were pleased to have had the opportunity to meet with the City -of Faith, Little Rock, Community Relations Board on the evening of March 13, 2000. It is evident that Director Terry Parker's commitment and work with this group has fostered better community awareness and understanding of the mission of Community Corrections and City of Faith. On Tuesday, March 14, 2000, Chief United States Probation Officer Claretha Nelson, United States Probation Officers Gary Duke, Joe Klingbough, Gary Duke, United States Marshal Conrad Pattillo, and Chief Deputy Bryan Murray visited City of Faith and participated in a meeting to discuss issues of shared interest and concern with the Bureau of Prisons reviewers, City of Faith Executive Staff and Case Managers. This was a productive and enjoyable meeting. Chief Nelson suggested having quarterly meetings with CCC staff and Probation to enhance communication. At 8:30 AM, March 15, 2000, the temperature of the hot water heaters was found to be 90 degrees Fahrenheit, although the temperature settings were 150 degrees. Chapter Three, section J of the Statement of Work states, "Showers and wash basins shall have hot and cold water. Hot water temperatures shall be thermostatically controlled at 120 degrees Fahrenheit. Corrective Action: Effective immediately, but by no later than April 29, 2000, the contractor shall ensure that the temperature of the hot water heaters is thermostatically controlled at 120 degrees Fahrenheit. 2. Chapter Four - Life/Safely a. The exit sign above the second floor south exit stairwell was not illuminating the letter "E" correctly. Chapter Four, section J., 2., of the Statement of Work states, "Externally and internally illuminated exit signs must be visible in both the normal and emergency lighting mode. " Corrective Action: Effective immediately, the contractor shall replace or repair the defective exit sign to ensure proper visibility of the sign. b. The door in the fire wall in the attic requires an automatic door closer to ensure the door remains closed at all times. Chapter Four, section A, of the Statement of Work states, "All NFPA 101, Life Safety Code requirements are considered mandatory. The BOP reserves the right to interpret and enforce all requirements of the version ofNFPA 101, Life Safety code under which the contract was awarded. " Corrective Action: Effective immediately, but by no later than April 29, 2000, the contractor shall install an automatic closer on the door in the fire wall of the attic. RECOMMENDATIONS It is recommended that City of Faith, Little Rock, prepare a "control tray" of the breakfast and dinner meals. In the event someone becomes ill from a meal, the examination of the control tray can enhance the investigation to determine if the food served was tainted. It is recommended that goggles and a paper respirator be placed in the medical kit in the event of a blood spill or blood pathogen emergency situation. Page 6 3. It is recommended that staff note in the "comments" section of the medication log that all pills were counted upon a resident's admission to the facility. Both resident and staff should sign the log verifying the count of the pills in each bottle. 4. It is recommended that the door to the womens restroom be adjusted to have a five -pound pull pressure. 5. It is recommended that the emergency evacuation diagrams on the second floor note that the "safe harbor" is located on the first floor. 6. It is recommended that copies of monthly telephone bills for all residents on home confinement be maintained in the file to serve as documentation that staff have ensured no unauthorized features have been added to residents' telephone service. COMMENDATIONS AND CLOSING COMMENTS The Community Corrections Office was very pleased to receive a communication from Discipline Hearing Officer David Ham, commending the staff at City of Faith, Little Rock, for the consistently excellent job they do on discipline cases. Mr. Ham stated the "excellent incident reports, investigations and CDC hearings make my job easy and is much appreciated". Great Job, City of Faith, Little Rock! Director Terry Parker and the staff at City of Faith, Little Rock, do an excellent job communicating information, reports, and other information in a timely manner to the Community Corrections Office. Introduction The audit of the City of Faith — Little Rock was conducted on June 26 - 27, 2000, by the following Team: James E. Dare, Chairperson; and Genie Powers, Member. The City of Faith — Little Rock was audited utilizing the Standards for Adult Community Residential Services, Third Edition and the 1998 Standards Supplement. This was the initial audit for the agency. B. Facility Description City of Faith is a non-profit organization founded by Mrs. Kathy Biedenharn in 1979. Ms. Biedenharn felt that the lack of money and marketable shills were the primary reasons why newly released prisoners failed to succeed outside the institution. As a result, City of Faith opened its first halfway house in 1983. The City of Faith operates three centers in the State of Louisiana and opened the City of Faith - Little Rock in 1996. The City of Faith — Little Rock Community Correctional Center is located at 1401 South Garfield Drive, Little Rock, Arkansas 72204. The center has a designed capacity for 97 residents, 77 male residents and 20 female residents. The present capacity during the audit was 58 males, 12 females, and 12 offenders on home confinement. The center provides services to adult offenders who are 18 years of age or older. The average length of stay is six months. The center receives direct referrals from the United States Federal Courts and the Bureau of Prisons. The security level for all residents is classified as minimum -community. The center is located in a residential/business community and has easy access to public transportation and also permits eligible residents to drive personal vehicles under stringent guidelines. The center is a two-story brick structure that has ten thousand (10,000) square feet. The center was built in 1996 and has two handicap parking spaces and forty parking spaces for staff and residents. The first floor of the center has a lobby, control station, administrative offices, kitchen, dining area, staff restroom, U.A./medical supply closet, director's office, laundry room, handicapped restroom, handicapped room (capacity for two residents), and four bedrooms for female residents that have alarmed doors at both ends of the hallway. The second floor of the center houses male residents. There are 13 rooms on the second floor that house between three to seven residents. There is a storage room, laundry room and restrooms on the second floor. There is a day room on the second floor that has ample room to sit and has access to a television. All resident rooms have access to natural light. All 2 common areas are personally decorated with plants, pictures and other memorabilia that add to the attractiveness and professionalism of the overall environment. The center is professionally landscaped and has several patios (covered and uncovered), barbecue area, three stationary picnic tables, several chairs and built-in sitting areas. The center has an outdoor (half -court) basketball area, and green -space for weight lifting. The center has a team of professionals that are divided into the following positions: 1 Executive Director 2 Deputy Executive Directors 1 Program Manager 1 Senior Case Manager 2 Case Managers 1 Chief of Security 6 Resident Security Staff 1 Home Confinement Coordinator The mission and management philosophy of the City of Faith Community Correctional Center is to provide a versatile community-based alternative to conventional incarceration for those offenders referred to the center by contractual agreements. Since every offender in residential treatment ultimately returns to the community, the center incorporates a carefully monitored, progressive reintegration into society. The City of Faith emphasizes a risk reduction goal utilizing programs designed to assist offenders in becoming law-abiding, self-sufficient, contributing members of the community; thereby, changing an offenders attitude and consequently the probability of future criminal behavior. Thereby, behavioral changes achieved during a relatively short period of residential treatment may become enduring characteristics that enable the offender to reintegrate safelyand productively into the community. The City of Faith offers a series of sentencing options more intrusive than probation, but less punitive than incarceration. Oversight, monitoring, substance abuse testing and counseling are provided. Thereby, maximizing compliance with treatment and court orders. The staff of the City of Faith males a concerted effort to confront unacceptable, inappropriate behavior, and the failure to meet program requirements. The Community Correctional Center in tandem with alternative to incarceration, offers a sensible long term solution to changing needs of the justice community. 3 C. C Pre -audit Meeting On June 25, 2000, the chairperson discussed the itinerary for the audit scheduled for June 26 - 27, 2000, with Kathy Biedenharn, Executive Director; Terry Parker, Deputy Executive Director; Ricky Banks, Deputy Executive Director; and David Beach, Accreditation Manager. The chairperson divided the standards into the following groups: Standard #'s 3 -ACRS -IA -01 to 3 -ACRS -3B-10 - Jim Dare, Chairperson Standard #'s 3 -ACRS -3C-01 to 3 -ACRS -5D-02 — Genie Powers, Member The Audit Process 1. Transportation The audit team was escorted to the center by David Beach, Deputy Director — Accreditation Manager. 2. Entrance Interview The audit team proceeded to the office of Terry Parker, Deputy Executive Director. The team expressed appreciation of the Association for the opportunity to be involved with the initial audit of the City of Faith — Little Rock Community Correctional Center. It was explained that the goal of the visiting team was to conduct a fair and thorough audit. The chairperson emphasized the goals of accreditation toward the efficiency and effectiveness of correctional systems throughout the United States. The audit schedule was also discussed at this time. The following staff were present for the Entrance Interview: Kathy Biedenharn Executive Director — City of Faith Ricky Banks Deputy Executive Director — City of Faith Terry Parker Deputy Executive Director — City of Faith — Little Rock David Beach Accreditation Manager 4 3 9 Facility Tour The audit team toured the entire center from 7:42 a.m. until 8:30 a.m. The following persons accompanied the audit team on the tour and responded to the team's questions concerning center operations: Kathy Biedenharn, Executive Director — City of Faith Ricky Banks, Deputy Executive Director — City of Faith Terry Parker, Deputy Executive Director — City of Faith — Little Rock David Beach, Case Manager — City of Faith - Monroe Throughout the tour, there was an atmosphere of professionalism and a mutual respect between staff and residents. The center was clean and staff offices were personalized with pictures and other items. Resident areas were clean and provided space for personal items. Overall, the center was well prepared and presented itself well throughout the tour. Conditions of Confinement/Quality of Life During the center tour, and upon revisiting each department, the overall impression and evaluation of the conditions of confinement and the quality of life were outstanding. Security The center is monitored by a well-trained staff. All residents sign in or out at the control room and are randomly searched for contraband and asked to submit to drug testing (urinalysis or breathalyzer). The security staff are well-trained in security techniques and monitoring resident activities to ensure_a high level of security. The center is considering installing security monitors for entrances, exits and common areas. Doors have alarms and are monitored at the control station. The center has emergency lighting and outdoor flood lights to enhance security. Environmental Conditions This center should be considered for one of the "Best In the Business" for the attention given to establishing an environment for success and an appreciation for quality furnishings. The audit team was thoroughly convinced that the environment (indoors and outdoors) contributes to the success of the center. 5 The overall environmental conditions of the center were outstanding. The center is to be commended for the outstanding documentation that established compliance with lighting (foot candles), air circulation, day and evening noise levels, room temperatures, access to telephones, and overall professional appearance of the center. Sanitation All _housekeeping, sanitation, maintenance and hygiene conditions met or exceeded standards. The center did not meet standards for lavatories, showers or wash basins; however residents can access restroom facilities 24 -hours a day and reported no problems to the audit team. Residents are assigned cleaning responsibilities twice a day to ensure cleanliness. Overall, the center maintains comprehensive housekeeping, maintenance and sanitation plans. This was documented with shift checklists and weekly sanitation and safety checklists. Fire Safety The center meets all standards applicable to ensure compliance with fire and safety regulations for staff and residents. All exit signs were operable, evacuation plans posted and fire extinguishers were tested according to schedule. There have been no violations of fire and safety codes. The center is to be commended for comprehensive monthly and quarterly fire/safety/sanitation inspection forms. The center has Edwards Systems Technology alarms and a sprinkler system. The system provides visual (flashing lights) and audible alarms. Food Service Baptist Hospital provides food service; however, there is a kitchen for residents to heat meals. There were vending machines for snacks and soft drinks on the covered patio area. Residents have access to an outdoor barbecue and can also have food delivered. The audit team ate dinner with the residents on Monday, June 26, 2000. The meal consisted of roast beef, mashed potatoes, blacked -eye peas, salad, dinner roll, and pecan pie. The meal was well-balanced, hot and nutritional. The hospital will also prepare special diets for any resident. The hospital provides a hot breakfast and dinner daily. A sack -lunch is available for residents, consisting of a sandwich, chips and dessert. Meals are also kept for residents who return to the center from work. iel Medical Care The center does not provide on-site medical care; however, staff are trained to provide emergency services. The center utilizes area hospitals, medical centers and dental services as a referral source for residents. All prescription medication is secured and residents are responsible for taking any medication. Residents are financially responsible for all medical care. Recreation The City of Faith — Little Rock has a half -court basketball court (outdoors), weight- lifting equipment, games, cards and television. Residents can also go to the War Memorial Gym on a social pass. The center also has an outdoor barbecue for recreational activities. Religious Programming City of Faith — Little Rock allows residents time to attend the church or synagogue of their choice in the community. There are several churches within waltzing distance of the center, or by using public transportation. The center allows ministers to visit residents at any time. Offender Work Programs Center case managers strongly encourage and believe that employment is an important aspect for successful adjustment to the community corrections program. The in-house work program ensures that all residents tape some responsibility for the cleanliness of the center. Academic and Vocational Education The case managers have current knowledge of what programs are available to assist the resident with educational or vocational training. The major focus is to secure gainful employment. Social Services The center case managers assist residents in referrals to the following area social services. The center contracts with Little Rock Mental Health to provide group counseling once per weep. The center also provides a 12 step program every Tuesday and Thursday. 7 Visitation The center provides and encourages visitation and developing a support system for each resident. Visiting hours are every Sunday from 12:30 p.m. to 4:30 p.m. Residents had no complaints about visitation hours, access to telephones or mail. The center also provides an opportunity to visit family with a daily social pass or weekend pass. Library Services Residents have access and are encouraged to use the public library or university law library. Laundry Residents have access to washers and dryers as well as ironing boards and irons. Residents can also use local laundry facilities. E. Examination of Records The audit team had ample space to examine all records/standards files. The center had no notices of non-compliance with local, state or federal laws or regulations. David Beach, Accreditation Manager, Terry Parker, Deputy Executive Director, Kathy Biedenharn, Executive Director, and Ricky Banks, Deputy Executive Director, and other staff were accessible to the audit team throughout the examination of standards files and additional observations of each department or resident living areas. The records were in good condition, both in content as well as the lay -out of each file. It was evident that a considerable amount of time/work was invested with the standards files. 1. Litigation Since entering the accreditation process, the center had no consent decrees, class action lawsuits or adverse judgments. 2. Significant Incidents City of Faith has had no incidents where use of force was used since 1996. The center noted six escapes on the Significant Incident Summary. The audit team commends the center for their staff -to -resident ratio and believes this reflects the success this community-based center is experiencing. 3. Departmental Visits The audit team visited the following areas during the center tour or throughout the audit to review conditions related to specific policies, procedures or operations: control room, case managers, offices, administrative offices, resident living areas and dining area. The audit team went in the Meld on two separate occasions: once to observe a home confinement visit and, second, to pick-up four residents from work. Each employee conducted themselves in a professional manner and operated center vehicles safely. 4. Shifts a. Day Shia (8:00 a.m. — 4:00 p.m.) b. Evening Shia (4:00 p.m. — 12:00 a.m.) C. Night Shia (12:00 a.m. — 8:00 a.m.) The audit team was present at the center during all three shifts. The audit team was able to interview several case managers and security staff and discuss their responsibilities for all three shifts. All staff interviewed indicated they feel safe in the center. One employee indicated that they felt safer in the center than they did on the street. Security staff conduct hourly counts and have multiple duties while covering the control station. F. Interviews During the course of the audit, the audit team was able to meet with staff and residents to provide direct observation to support secondary documentation. 1. Offender Interviews During the course of the audit, the audit team was able to interview 25 residents. Throughout the interviews, residents conveyed that staff were accessible, that they felt the programs and opportunity to work were bene'fi'cial and that they were treated with respect. There were no major complaints from residents throughout the audit. Residents spoke highly of the center director and the professional atmosphere witliln the center. 2. Staff Interviews There sere 13 staff that were interviewed throughout the audit. The overall impression that staff conveyed to the audit team was that they enjoyed their work and 9 they were able to do their jobs. It was refreshing to see such a high level of camaraderie. Management and line staff have a very good working relationship. The audit team observed the shift change. 3. Files Examined (Resident and Personnel) All residents and staff signed a release of information form giving the audit team permission to review their file. The audit team reviewed six personnel files. The personnel files were extremely well -organized and had a table of contents for easy access to information. The audit team reviewed ten resident files that are maintained by the center case managers. The resident files were comprehensive and had a resident file index that divides the file into 29 sections for easy access to information and accountability for the center. G. The chairperson welcomed and acknowledged the staff of City of Faith — Little Rock for their support and hospitality throughout the audit. The chairperson explained the time frames for the preparation of the visiting committee report and the process leading up to the accreditation hearing. The chairperson reported that the findings 10 of the audit team were preliminary and that the final compliance rating would be made by the commission on accreditation for corrections. The audit team identified the following notable achievements by the City of Faith — Little Rock: 1. The center has an environment that captures the true mission of corrections and rehabilitation. 2. The center has designed an excellent resident handbook. 3. The center had spent many hours preparing and highlighting primary and secondary documentation. The red files made it easy to identify mandatory standards. 4. The center has excellent community support which was evident by the attendance at the exit interview and letters of support from the chief of police, United States Federal Court, neighborhood association and city director. 5. The center is to be commended for making reasonable accommodations for residents who are physically challenged. 6. The City of Faith has had no employee grievances filed since 1997. 7. The security staff are well-trained at covering the control station and ensuring accountability of residents and overall safety and sanitation of the center. 8. The center prepared an emergency evacuation plan that the city emergency management specialist reported should be a model for other agencies. 9. The center has had no use of force incidents since the center opened in 1996. 10. The administration is to be commended for setting a high level of expectations for staff and residents. The audit team expressed their thanks to David Beach for his assistance throughout the audit and to the entire staff and residents for their hospitality. 11 COMMISSION "ON ACCREDITATION FOR CORRECTIONS City of Faith — Little Rock Little Rock, Arkansas June 26 - 27, 2000 Visiting Committee Report Non -Mandatory Standards Non -Compliance Standard # 3 -ACRS -2C-05 VISITING COMMITTEE AUDIT - Finding Non -Compliance THE FACILITY HAS, AT A MINIMUM, ONE OPERABLE TOILET FOR EVERY 10 OFFENDERS IN MALE FACILITIES AND ONE FOR EVERY EIGHT OFFENDERS IN FEMALE FACILITIES. URINALS MAY BE SUBSTITUTED FOR UP TO ONE-HALF OF THE TOILETS IN ALL MALE FACILITIES. At full capacity, the center has one operable toilet for every 15.4 male residents and would have to add three toilets for compliance. The center has one operable toilet for every twenty female residents and would need to add two toilets for compliance. Residents reported no difficulty accessing restroom facilities. Waiver City of Faith — Little Rock does not have ample space for renovation or modification and would place an undue hardship on the center. AUDITOR'S RESPONSE The visiting committee did not find the present ratio of residents to toilets to impact the quality of life or access to toilets. Residents have access to restroom facilities 24 - hours a day. It is recommended that the plan of action waiver be approved. 12 Standard # 3 -ACRS -2C-06 VISITING COMMITTEE AUDIT - Finding Non -Compliance THE FACILITY HAS, AT A MINIMUM, ONE OPERABLE WASH BASIN WITH HOT AND COLD RUNNING WATER FOR EVERY SIX OFFENDERS. At full capacity the center has one operable wash basin for every 9.6 male residents and needs to add live wash basins for compliance. The center has one operable wash basin for every ten female residents and would need to add two wash basins for compliance. Waiver City of Faith — Little Rock does not have space to add additional wash basins and it would also add an undue hardship on the center. AUDITOR'S RESPONSE The visiting committee did not find the present ratio of wash basins limited access or impacted the quality of life. It is recommended that the plan of action waiver be approved. Standard # 3 -ACRS -2C-07 VISITING COMMITTEE AUDIT - Finding Non -Compliance REVISED AUGUST 1995. INMATES HAVE ACCESS TO OPERABLE SHOWERS WITH TEMPERATURE -CONTROLLED HOT AND COLD RUNNING WATER, AT A MINIMUM RATIO OF ONE SHOWER FOR EVERY EIGHT INMATES, UNLESS NATIONAL OR STATE BUILDING OR HEALTH CODES SPECIFY A DIFFERENT RATIO. The center has one shower for every ten female residents and needs to add one shower for compliance. AGENCY RESPONSE Waiver City of Faith — Little Rock does not have space to add additional showers and it would create an undue hardship on the center to add additional showers. AUDITOR'S RESPONSE The visiting committee did not find that the present ratio of showers limited access to residents nor impact the quality of life for residents. The showers provide privacy because they are individual pre -molded units. Residents are required to be clean shaven. Beards are allowed as long as they are kept neatly trimmed and are of reasonable length. Residents are required to keep their nails clean and closely clipped. Residents are not permitted to wear unacceptable headwear which includes, but is not limited to: do -rags, shower caps, skull caps or bandanas. Residents are not allowed to wear revealing or provocative clothing. Residents are required to wear their clothing in an acceptable manner. In addition, to facilitate the cleanliness of the entire facility and its surrounding area, residents will be assigned, on a rotating basis, janitorial and/or maintenance responsibilities. A written assignment schedule will be posted in a conspicuous location within the facility. EMERGENCY EVACUATION PROCEDURES -Cit} of Faith conducts, monthiy fire acid emergency evacuation drills. Evacuation routes are posted throughout the facility and all emergency exits are clearly marked. Residents will be instructed on the particulars of City of Faith's emergency evacuation procedures during their orientation. EMPLOYMENT GUIDELINES Residents are required to maintain full-time employment. Full-time is defined as at forty (40) hours per week. For residents attending either academic or vocational school, the Director will determine the appropriate number of hours to be worked. Any employment obtained by residents must pay at least minimum wage. Residents are allowed to seek employment within a hundred (100) square mile radius of City of Faith. All employment must be approved by the Director and verified by the Federal Case Manager. Residents may not change employment without prior approval of the Director. Residents who are terminated from their job due to unsatisfactory performance, poor attitude, or misconduct may incur sanctions including being returned to the custody of the Bureau of Prisons. Residents are not allowed to work at an establishment which sells alcoholic beverages as its primary commodity. Residents have fifteen (15) working days in which to obtain employment. FURLOUGHS AND PASSES Other than employment or a City of Faith sanctioned and escorted trip, a resident is allowed to be absent' from the facility only as a result'of the following: PASS. Such Committee only reviews violations which fall under the major and/or minor categories. Terminal violations are handled exclusively -by the Federal Bureau of Prisons. IN-HOUSE DISCIPLINARY RULES AND SANCTIONS All -City of Faith residents are, of course, required to comply with the Federal Bureau of Prisons Prohibited Acts during 'the course, of their entire stay. In addition, City of Faith maintains In -House. Disciplinary Rules which all residents are also required to strictly adhere to. In an effort to ensure that residents know what is expected of them as well as penalties for non-compliance, violation are divided into three categories: terminal, major, and minor. Categories, violations under each category, and correlative sanctions are discussed below. TERMINAL CATEGORY: rk Terminal Category Violations may subject a resident to immediate termination from City -of Keith s Wal Release Program. City of Faith; irrimediately upon gaining knowledge of a violation within t Category, will contact the Bureau of Prisons Community Corrections Manager, appropriate Federal Parole Officer, and/or social law enforcement agencies both Federal and State, if the situation so warrants. Terminal Category Violations which will subject a resident to immediate termination are: 1. ARSON. Setting or attempting to set a fire. 2. CONTRABAND. Possession and/or distribution of firearms, knives, sharpened instruments or other weapons. Unauthorized possession or distribution of any narcotic, drug, or intoxicant not prescribed for the resident by medical personnel. Any resident who fails a drug test and/or breath test may be considered in violation of this rule. 3. ESCAPE. Leaving or being absent from City of Faith without authorization and whereabouts unknown. Leaving or being absent from a job site or unauthorized place without permission with whereabouts unknown. Attempting, planning or conspiring any of the above. 4. EXTORTION. Threatening another with bodily harm. Demanding or receiving anything of value, either articles or services, in return for protection from injury or other considerations. 5. FIGHTING. MAJOR CATEGORY: Major Category violations could.possibly result in a resident's termination, but normally results in a resident incurring severe sanctions. Major Category Violations which may subject a resident to termination or severe sanctions: 1. BRIBERY. Bribing or attempting to bribe any staff member. 2. DISOBEDIENCE. Disobeying posted policies, directives, direct verbal orders, or instructions from staff. When orders conflict, the last verbal order must be obeyed. 3. DISRESPECT. Displaying and/or voicing any verbal and/or nonverbal disrespectful action towards a staff member or another resident. Directing profanity towards staff or resident is an example. 4. FIGHTING. Fighting which results in no or only minor injury. Minor injury is defined as an injury which can be handled with in-house first aid. 5. PROPERTY DESTRUCTION. Minor destruction or damage to City of Faith property and/or another resident's personal property either willfully or through gross negligence. Smoking outside in areas not designated for such purpose. 4. UNAUTHORIZED AREA. Being in an unauthorized area without staff permission. 5. UNAUTHORIZED CONTACT. LAUNDRY City of Faith provides a separate laundry facility adjacent to the Center which contains two (2) washers, two (2) dryers and three (3) vending machines. The laundry facility opens at 5:00 am and closes at 10.30 piri Sunday through Thursday, while staying open until midnight on Fridays and Saturdays. All residents are, expected to treat all laundry equipment and vending machines with respect and care. Also, residents are responsible for ensuring their laundry is removed promptly after either washing or drying to enable all.residents to make.optimum use of the laundry facility. Residents are responsible for providing their own laundry detergent. 9. Each resident will be issued the following items upon their arrival at City of Faith: 1.(1) laundry. bag 6. (1) wash cloth . 2. (1) pillow case 7. (1) plastic glass 3. (1) fitted sheet 8. (1) roll toilet paper 4.(1) flat sheet 9. All resident rooms contain an approved fire -retardant trash can. This can is not to be removed, nor is any other type of can to be substituted. City of Faith provides each resident with a fire -retardant pillow upon arrival, Personal pillows are not allowed and will be immediately confiscated if found. Room assignments will -be issued without regard to age; race, religion, or. creed. All resident requests for room changes -are to be directed to the Security Supervisor. MAIL City of Faith maintains the option to randomly monitor resident's mail, however, such monitoring will only be done in the presence of the resident whose mail is being investigated. MEALS City of Faith. provides residents with a hot and nutritious breakfast and diririer, with the noon meal being facilitated by means of a sack lunch. Sack lunches are distributed during breakfast. The meal schedule is listed below. Residents have access to review their records at the discretion and only in the presence of the Director, and then only those records allowed by the Bureau of Prisons to be reviewed. REMOVAL OF A RESIDENT UPON REQUEST Upon the recommendation of the Director, any resident may be removed from City of Faith for proper cause and immediately returned to the Bureau of Prisons. Failure of a resident to be at a location for which he/she has signed out for, may cause such resident to be considered on escape status with appropriate measures being immediately invoked. ACCESS TO COURTS: Residents have the right to present any issue, including the following: Challenging the legality of their convictions or confinement, seeking redress for illegal conditions or treatment while under correctional control, pursing remedies in connection with civil legal problems; and asserting against correctional or other government authority any other rights protected by Constitutional or statutory provision of common law. Residents seeking judicial relief are not subjected to reprisals or penalties because of the decision to seek such relief. Residents seeking relief must do so in times which will not interfere with work hours or other program requirements. PROTECTION FROM HARM: Residents are not subjected to corporal or unusual punishment, humiliation, mental abuse, or punitive interference with the daily functions of living, such as eating or sleeping. GRIEVANCE PROCEDURES: A written offender grievance procedure is made available to all residents. Administrative remedy procedures are posted on bulletin boards. Monroe, LA isn't on the coast but always feels the affect of the Atlantic Hurricane season each year. Some people head north while others, City of Faith residents, don't have such an option. Instead, they volunteer. City of Faith will be partnering yet again with the City of LR Alert Cen- ter Facilitators to co -host the 2012 Silver Linings Project. This project was cre- ated and is coordinated by City of Faith staff and residents. Each year COF-LR is able to feed and give gifts to over 300 senior citizens in the Little Rock area. So much so, City Director Joan Adcock had to take a few weekends off Ottenheimer from the LR Animal Village and meet the vol- unteers at Wakefield & Ottenheimer Parks. Parks "We took 2 weeks off from the Animal Village to give back to some of the great neighbor- hoods in Little Rock. The City of Faith residents never complained about the heat.
