SURVEYOR'S NOTES
1. This is a Rooftop Svrvey, made under the supervision of an Arkansas Registered Land
Surveyor. Dote of Field survey is Septernbar 14. 2004.
2. Bearings are based on Arkansos South State Plane Coordinates NAD 83.
3. No underground utilities, underground encroachments or building foundations were
measured or located as a port of this survey, unless otherwise shown. Trees and shrubs
not located, unless otherwise shown.
4. Benchmark used is a concrete monument and brass disc. NAVO 88 Datum with an
established elevation of 369 feet, (112.5 meters), PID AE3002. Onsite benchmark is as
shown hereon. Elevations shown are in feet and refer to NAVD 196.
5. This survey was conducted for the purpose of a Rooftop Survey only. and is not
intended to delineate the regulatory jurisdiction of any federal, state, regional or local
agency, hoards commission or Other similar entity.
b. Attention is directed to the fact that this survey may have been reduced or enlarged in
size due to reproduction. This should be taken into consideration when obtaining scaled
data.
7. This is a Rooftop Survey only, no property or government survey linos ore a or
mode a part hereof. Surveyor is unable to address any title commitment or title ced ertiRcate
pertaining to this sufvey.
19. Surveyor hereby states the Geodetic Coordinates and the elevation shown for the
centerline of the tower are accurate to within +/- 20 feet horizontally and to within +/-
5 feet vertically (FAA Accuracy Code 1A)•
9. Survey shown hereon conforms to the Minimum Requirements as set forth by the State
Board for a Class 'A' Survey.
10. Meld data upon which this map or plat is based has a closure precision of not less
than one -foot in 15.000 feet (1';15,000') and an angular error that does not exceed 10
seconds times the square root of the number of angles turned. Field traverse was not
adjusted.
11. No field measurements were used to determine the location of the door or University
Avenue as described in the legal description for the Ingress/Egress Easement.
12' INGRESS/EGRESS EASEMENT
An easement lying in Section 31, Township 2 North, Range 12 West, Pulaski County. Arkansas and
being more particularly described as follows:
Beginning at the front door of 1501 North University Avenue Found On the South side of the
building; thence run southerly a distance of 6.00 feet to a point; thence run westerly 300 feet,
more or Fess to University Avenue and the Point of Ending. Said described easement being 12 feet
in width and lying 6 feet each side of the said described centerline.
FLOOD NOTE
The subject property appears to lie in Zone Y' of the Flood Insurance Rate Mop Community Panel
No. 050181 0064 r, which bears an effective dote of October 19, 2001 and IS NOT in a special
flood hazard area. No field measurements were used in this determination and an elevation
certificate may be needed for verification.

accordance with `Minimum Standard Detail Requirements for ALTA/ACSM Land Title Surveys.` jointly
established and adapted by ALTA. AGSM and HSPS in 1999, and includes Items 1-6. 10-12 of
Table A thereof. Pursuont to the Accuracy Standards as adapted by ALTA, HSPS, and AGSM and in
affect on the data of this certification, undersigned further certifies that;

All hardware required to mount the requested antennas, coax, base station equipment and
equipment cabinets will be provided by applicant, at applicant's cost, by the original
manufacturer of the roof. Hardware not approved by the roof vendor will not be allowed.
Prospect Building will have final approval of any vendor used for installation, construction or
roof modification work. Applicant will furnish, for the approval of Prospect Building, all
installation, construction or roof modification documents. Prospect Building will have the right
to perform a final inspection of any work performed and will have the right to require punch list
corrections.
Applicant is responsible for and agrees to pay all costs associated with securing all applicable
permit approvals required for applicant's proposed operations. Applicant must provide Cm -Net
with copies of all permit approvals prior to Prospect Building's approval of and the
commencement of any work associated with any approved co -location.

operating a wireless telecommunications facility. It is my understanding that in -order to utilize
the rooftop for the intended purpose, Verizon Wireless will need to apply for permit approvals
with the City of Little Rock.
Please accept this letter as my/our approval for Verizon: Wireless to file any and all necessary
applications for permits or approvals in order to proceed with Verizon Wireless' intended use of
the rooftop to build and operate a wireless telecommunications facility on our property.
