BMD Builders has been approved as authorized agent for Hut Little Rock, LLC. to demolish the
above referenced property. I have included the application for certification of appropriateness as
well as BMD's letter of authorization. Also included are elevation photographs of referenced
property, supporting documentation from ATOKA Inc, showing negative test results for Asbestos,
BMD's filing fee and All property owners within a 200' radius.
