A complete application is needed before close of business on Monday, February 13, 2023 for items to be placed on the April agenda. 1.       Cover letter – explain why you are requesting the CUP. 2.       On the application provide: R-3 for zoning, CUP requested…Duplex(?), provide a response for use restrictions and you or your agent must provide a signature – if an Agent will represent the application, a notarized affidavit must be provided. 3.       Complete the Street Right-of-Way Agreement – be sure to answer the questions.  I’m not sure how to respond to the bill of assurance. That office is where applicants are provided to citizens. You may want to call them back. Finally, if memory serves me correctly, the existing structure on the lot will not be razed and you are proposing to convert it into a duplex. Please clarify. The survey has already been completed and paid for. I should receive a copy of it today or tomorrow. Once received I will email it to you. I called the city circuit clerk's office and requested a bill of assurance and they did not know what I was talking about? Not sure, what or who else I need to contact for that information. I have mailed out a check to the department of planning for $180 attention to you. Please let me know what else I need to do.  Thank you, Sincerely, Cole
