Dear Mr. Carney:
Please accept this letter as a formal request and application for administrative approval for the proposed
Wireless Communications Facility, pursuant to City of Little Rock Municipal Ordinances, Section 36-
594. In connection with the proposed raw land site upon which Verizon Wireless will construct a one
hundred fifty (150) foot monopole located at 10402 Stagecoach Road, Little Rock, Arkansas, please find
the following enclosed documents which make up the administrative review application:
1. Check for Payment of the Application Fee: $100.00;
2. Wireless Communication Facility Application;
3. Copy of the notarized Letter of Authorization;
4. Written Application Statement;
5. Co -location Statement;
6. Statement of Radiated Power;
7. Four (4) Scaled Site Plans; and
8. Two (2) Surveys of the Leasehold.

Wireless Communications Facility Application Statement
Verizon Wireless Tennessee Partnership, d/b/a Verizon Wireless (the "Applicant")
hereby requests approval for the construction of a Wireless Telecommunications Facility
(the "Facility"), pursuant to the City of Little Rock Municipal Ordinance, Article XII,
specifically Sections 36-593: Development standards, 36-594: Review process, and 36-
595: Approval procedures.
The proposed Facility will be used for the transmission of wireless telecommunication
signals and will be located on a 30' by 35' parcel containing 1,050 square feet (the
"Lease Space") that Applicant has leased from Otter Creek Village Commercial, LLC
(the "Landowner"). The Property is located at 10402 Stagecoach Road, Little Rock,
Arkansas.
Applicant respectfully submits that it should be granted approval for the Facility
administratively since it fully complies with the development standards set forth in
Section 36-593. In addition to promoting the local application of its Federal
Communication Commission license, the Facility will protect the public health, safety
and welfare by providing telecommunications coverage to areas that currently have no
coverage.
The proposed Facility fully complies with the provisions of the Ordinance applicable to
Wireless Communication Facilities requirements since it is a 150' monopole, and set
back 59.2' from the nearest property line. The proposed Facility is clean, quiet and
produces no odor, dust or glare. Furthermore, the Facility does not create excessive
demands for sewerage, water, gas, electricity and similar services. The proposed Facility
complies with the landscaping and screening requirements set forth in the Ordinance.
Given that the Applicant is in compliance with the Ordinance and its development
standards, the Applicant respectfully requests that it be granted administrative approval
for the construction of the Facility.

In compliance with the Little Rock Municipal Code, Section 36-593, Benson Brown hereby
certifies that search attempts were made to find sites in the specified need area in which the
possibility of co -locating on an existing tower and/or attaching to an existing stricture would be
possible. There were no structures providing an opportunity for co -location within that
placement area able provide adequate coverage to the area in need.
In addition, Benson Brown hereby certifies that this facility, as planned, can accommodate no
less than two (2) additional antennas arrays and ancillary equipment, that Verizon Wireless will
welcome collocation agreement requests for this site from other carriers, competitors and non -
competitors alike, and will accept such agreement at prevailing market terms and conditions.

The Equivalent Isotropically Radiated Power (EIRP) for this installation is 890 watts or less. The
current FCC rule (reference: Code of Federal Regulations, Title 47 — Telecommunications,
Chapter 1- Federal Communications Conunission, Part 24- Personal Communications Services
Subpart E — Broadband PCS, Section 24.232- Power and antenna height limits) permits up to
1640 watts EIRP for installations of this type.
This Installation as designed meets the FCC Requirements.
