I am submitting an Application for a Conditional Use Permit for the referenced commercial development.

2. Three (3) copies of a survey of the referenced property.
3. Application for Conditional Use Permit
4. CUP R-O-W Dedication Agreement
5. Affidavits from the property owners designating me as their agent.
6. Copies of the Notices of Public Hearing mailed to adjacent property owners.
