RE: Lei Clinic Short Form POD Application
Dear Monte:
Enclosed are the following items regarding our application for the above Planned Office
Development application:
1. Three copies of the current property survey.
2. Eighteen copies of the POD Site Plan.
3. Completed CLR application forms.
4. Owner’s affidavit and right-of-way agreement.
5. Our check for the $230.00 filing fee.
This project proposes to develop a multi-use building of approximately 8,800 SF,
comprised primarily of a medical clinic, and also including general office and a limited
amount of restaurant space with no drive-through. The building is proposed to be
partially one story, with a second story portion on the northern end of the building.
We are requesting approval of two waivers regarding the Highway 10 DOD standards:
1. Allow reduced side yard buffers on portions of the eastern boundaries of the site.
2. Allow reduced side yard buffers on portions of the western boundary of the site.
The overall average of side yard buffers easily meets the requirements. We are
requesting reductions in two areas to better fit the project onto this site with its triangular
shape and steep grade.
