2. What type of food will be served?
3. Days and hours of operation?
4. Site plan or survey indicating location of food truck?
5. Is operator the owner of the property or is a lease agreement in place,
6. Provide terms of lease agreement?
7. Is parking and vehicular circulation adequate?
8. Will food truck relocate or remain at this location at all times?
9. Will the vehicle be permanently connected to utilities?
