On behalf of Marlow E. Ball, D.V.M. PLLC ("Applicant"), I am submitting an
application for a Certificate of Appropriateness (COA) along with other required
submittals and this cover letter requesting approval for completion of permanent signs
on an existing buildinlocated on the North West corner of Scott and 13th Street
designated 200 E. 13t Street, Little Rock, Arkansas ("Property"). The Property is
owned by Alessi -May LLC ("Property Owner") and leased to Applicant, who operates a
small animal veterinary clinic there under the name of Soma Animal Clinic &
Housecalls.
The application is for two signs to be attached to the building. One sign is to be
on the South side of the building facing 13th Street and the other on the West side of the
building facing Scott Street as shown by the submittals included.
The signs are identical in size, however the content of the signs may vary. (See
Submittals Exhibit 2. B (2) pages 5 and 6).
1. The completed application form and Authorization of Representation Statement
from the Property Owner to the Applicant is included with this cover letter.

2. Other Submittals included:
A. New Construction, Additions or Modification to Existing Structure, Moving
a Building into the District
(1) Scaled drawings. No scaled drawings are submitted. Since the
project proposed is for attachment of signs to an existing building there
will be no new construction, additions or modifications to existing
structures. The photos submitted with the application adequately
depict the project proposed (See Submittals Exhibit 2. B.), and
Applicant believes no scaled drawings are required. If you feel
otherwise, please advise me immediately.
(2) One set of scaled drawings on 24" x 36" and ten copies of the drawing
on 8 '/2" x 14 or 11" x 17". See 2. A. (1) above. No scaled drawings
are submitted as the photos included with the application adequately
depict the project proposed (See Submittals Exhibit 2. B.), and
Applicant believes no scaled drawings are required. If you feel
otherwise, please advise me immediately.
(3) All building materials described in the Design Guidelines should be
noted on the scaled drawings. No scaled drawings are submitted as
the photos included with the application adequately depict the project
proposed (See Submittals Exhibit 2. B.). A description of materials to
be used is set forth below. See 2. A. (4) below. Accordingly, Applicant
believes no scaled drawings listing building materials are required. If
you feel otherwise, please advise me immediately.
(4) A description of material for all exterior surfaces are:

DiBond 3mm with cut or printed vinyl applied

Size — Height 42 inches
Width 140 inches
Direct mount onto building with steel, stand-off hardware Installed into
mortar joints.

B. Photos

C. Materials to be used:
No samples are submitted. The materials for the project are described
above. See 2. A. (4) above. Applicant believes the project is adequately
depicted in the photos submitted. If you feel otherwise, please advise me
immediately.
D. For Demolition Only. No demolition is anticipated and accordingly, there
are no demolition submissions.
E. Additional Documents. Please advise me immediately if you require any
additional documents for clarification and review so that they can be
submitted no later than three weeks before the meeting date.
3. Proof of Ownership. A. A copy of Special Warranty Deed dated February 18th
1997 and filed of record March 7, 1997 as Instrument No. 97 014802 is submitted.
as proof of ownership of the Property by Alessi -May LLC. (See Submittals Exhibit
3 A. Special Warranty Deed)
B. Lease Agreement. A lease agreement between Marlow E. Ball, D.V.M. PLLC
and Alessi -May LLC is submitted. The Lease Agreement has been redacted to
preserve confidential proprietary information of the Applicant. (See Submittals
Exhibit 3 A. Lease Agreement).

A notarized statement of the project Property Owner's consent for the Applicant to
pursue the COA is submitted. (See Exhibit 3-13 (2) Property Owner's Consent for
Applicant to Pursue this COA).
4. A. A list of all property owners located within 200 feet of the subject property
certified by Stewart Abstract Company dated January 15, 2019 is submitted. (See
Submittals Exhibit 4).
Applicant has no knowledge of any other property owners that are not reflected on
the Stewart Abstract Company list but will add any property owners not on the list
to the list should Applicant learn of any such property owners.
B. Applicant will give at least 10 days written notice of the time, place and date of
the public hearing to the property owners on the list and any other property
owners Applicant obtains knowledge of by certified mail return receipt requested
as required in the instructions once the application has been accepted, a file
number has been assigned, and a date set for the public hearing with the form
"Notice of Public Hearing" with the address on the back of the PS Form 3811:

C. Applicant will sign and complete the Affidavit at the bottom of the "Notice of
Public Hearing" and return to Staff as soon as possible.
5. Five business days prior to the meeting, Applicant will submit:
A. Green and white certified mail receipts (PS Form 3800 proof of mailing),
stamped by the post office,
B. Certified List of Recorded Property Owners Notified
C. List of Owners Notified and,
D. Copy of the completed public meeting notice and singed Affidavit.
6. At least 10 days before the hearing, Applicant will post the subject property with
signs to be furnished with one sign facing Scott Street and one sign facing 13th
Street.
7. The Applicant will be present at the public hearing in order to answer questions
the Commission or interested parties may have.
