A RESOLUTION AWARDING A CONTRACT FOR ACCIDENT
INSURANCE FOR PARTICIPANTS IN THE MUNICIPAL
SPONSORED RECREATIONAL PROGRAM TO THE EXTENT
THAT SUCH IS REQUIRED BY THE NATIONAL LITTLE
LEAGUE ASSOCIATION
BE IT RESOLVED BY THE BOARD OF DIRECTORS OF THE CITY OF LITTLE
ROCK, ARKANSAS .
SECTION 1. The bid of Mutual of Omaha, Marquis E Jones, Agent
attached hereto and made a part hereof the same as though set out
herein word for word submitted in keeping with plans and specifi-
cations on file in the office of the Purchasing Agent to provide
accident insurance to the participants in the municipal sponsored
recreational program for the remainder of this summer to the extent
that such is required by the National Little League Association is
hereby accepted and contract is hereby awarded to the said successful
bidder to provide such insurance at a cost not to exceed Eight
Hundred Seventy Five ($875-00) Dollars
SECTION 2 . The funds to defray the cost of the premium provided II
for in Section 1 hereof shall be paid totally and only from such funds
as may be received from the Federal Government and not from tax revenues
of the City of Little Rock.
SECTION 3 . This Resolution shall be in full force and effect
from and after its adoption.

Dear Mr. Duke:
We are happy to offer the following as our bid for the 800 participants
in your summer recreation program sponsored by your organization:
$5,000.00 Accident Medical Expenses with no deductible
$1,000.00 Accidental Death and Dismemberment -----
Coverage is on a non-enrollment basis, and is to run for 90 days from
effective date. Policy does not include coverage for football or for
hockey.
The premium is $1,400.00 and policy is effective as of 12:01 Noon on the
date of full premium payment. A policy evidencing such coverage will be
