11830I 2 RESOLUTION NO. 11,830 3 A RESOLUTION TO AUTHORIZE THE CITY MANAGER TO 4 RENEW THE CONTRACT FOR EMPLOYEE DENTAL 5 INSURANCE WITH DELTA DENTAL PLAN OF ARKANSAS 6 FOR AN ADDITIONAL YEAR; AND FOR OTHER PURPOSES. 7 8 WHEREAS, Delta Dental Plan of Arkansas has submitted a proposal to renew 9 the existing dental care policy for City employees for 2004 at no increase in premium 10 nor any change in benefit structure, and 11 WHEREAS, the Request for Proposal issued in September, 2002, included a 12 provision to allow renewal of the policy at the City's option based on premium and 13 service provided, and 14 WHEREAS, Delta Dental Plan of Arkansas has been the provider of coverage 15 since the inception of the City's program to provide dental care coverage for City 16 Employees and on each occasion of bidding has offered the best combination of 17 coverage and premium rates. 18 NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF 19 DIRECTORS OF THE CITY OF LITTLE ROCK, ARKANSAS: 20 Section 1. The City Manager is authorized to renew for the year 2005 an 21 agreement with Delta Dental Plan of Arkansas to provide dental insurance coverage for 22 eligible City employees with no increase in premium nor change in benefit structure. 23 Section 2. The City Manager is authorized, if deemed in the best interests of 24 the City, to extend this contract for a period of one year in accordance with the terms set 25 forth in Delta Dental proposal of September, 2002. 26 H Resolution [PAGE 1 OF 21 Dental Insurance Contract 1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 Section 3. Funds for the cost of this coverage will be included as a line item in the 2005 City budget. Section 4. Severability. In the event any portion of this ordinance is declared or adjudged to be invalid or unconstitutional, such declaration or adjudication shall not affect the remaining portions of this ordinance, which shall remain in full force and effect as if the portion so declared or adjudged invalid or unconstitutional was not originally a part of this ordinance. Section 5. Repealer. All ordinances and resolutions, and parts thereof, which are in conflict with any provision of this ordinance are hereby repealed to the extent of such conflict.
